Our Additional Primary Care (APC) service is looking for a regular team ofConsultantGPs who willbe responsible forproviding supervision toourmulti-disciplinary teamof non-medicalprescribers andacting as a source of clinicalexpertiseto the team, enhancing skills and knowledge.
Triage to,and supervision of,our NMP team is an integral part of this role.
Main duties of the job
The post holder will autonomously undertake assessment and treatment ofbothacuteandchronic,differentiatedand undifferentiated, patient presentations.
The post holder willdeterminea clinical diagnosis, ifappropriateand treatment plan and appropriate discharge, or refer,asindicated,whilstmaintainingaccuratepatient records and ensuring patient safety. They will be a source of clinicalexpertiseto less experienced staff, enhancing skills and knowledge, and work in partnership with a multidisciplinary team.
Hours of work:8am-8pm Saturdays and Sundays (Flexible hours negotiable)
Tenure:Flexible - Fixed term contract to 30th June 2027 with potential extension (zero hour contract considered
Base:Hybrid working. Predominantly remote working with some requirement to come to the ABC offices at Crawley and Horley.

Clinical responsibilities include:
- Triaging patientappointmentsandallocatingthem to the mostappropriate clinician.
- To provide clinical supervision to all members of the multidisciplinary team working within the APC service
- Patient consultations
- Issuing medication
- To independently undertake specialised clinical assessments, diagnose,treatand discharge, or appropriately refer, patients with a wide range of acute and chronic illnesses
- To ensure patients are fully involved in the design and implementation of their treatment plans
- To adapt styles of communication appropriately to promote empowerment, self-management, and health education for patients
- To attend clinical meetingsifrequired
- To be a source of clinicalexpertiseto less experienced staff within the team
- To delegate duties/tasks to other members of staff where appropriate
- Toidentifyactual or potential clinical risks and takeappropriate actionusing, whenrequired, theappropriate reportingstructure
Leadership / Service Delivery responsibilities
1. To ensure that patient records and documentation aremaintainedin line with recognised standards and ABC policies and use of the electronic patient records system. This includes
- Documenting full andaccuraterecord of assessmentsandtreatment
- Supporting colleagues tomaintaingood and complete records.
- Working closely with the administration and/or operations team, ensuring all records pertinent to patients are filed asappropriate.
- Contributing to the read-coding of patient data.
2. Work within local and national guidelines and protocols
3. Toparticipatein audits and use the results, currentresearchand government guidelines, to make recommendations for changes to service delivery and clinical practice, and to share knowledge with other healthcare professionals.
4. To collect data and provide service information to the Service Manager asrequired.
5. To contribute to the development and establishment of systems to gather feedback from patients.
6. To ensure thatstakeholdersare kept informed on service issues and potential risks.
7. To work with the team in delivering and promoting the needs of the service.
8. To work with the Service Manager and Clinical Lead to meet the clinical governance needs of the service; including investigation of incidents (including serious incidents) and complaints, whererequired, sharing learning that arises from investigations.

3. To support an ongoing programme of audit to evaluate practice ensuring that the data is used to develop and improve the service.
4. Support multi-disciplinary clinical audit and research across theservice implementinginnovations in clinical practice and supporting the utilisation of research andevidence-based practice.
5. Support quality improvement and the use of methodologies to promote good clinical practice.
6. Using computer software to input, extract and analyse data as required and to develop and create divisional/specialty reports and other reports asrequired.
