Job summary
This is an exciting expansion to our PCN Children and Young Persons Wellbeing Service, and as such the successful post holder will have the opportunity to be involved in the development of the service, research opportunities, as well as having the responsibility for collecting user feedback and adapting the role to suit the needs of users in line with the THRIVE model and Goal Based Outcomes (GBO) Framework.
The Health and Wellbeing Coach will be part of a network-based Wellbeing Team including a Social Prescribing Link Worker to engage with children, young people and families and deliver a combination of one to one, group, and virtual interventions to improve their mental health and wellbeing.
The successful postholder will use their coaching skills to support people to develop the knowledge, skills, and confidence to manage their own health and wellbeing, whilst increasing their ability to access and use community support offers. You will work as part of a small team, undertaking initial assessments and delivering a range of evidence-based interventions, which will include holding a caseload.
Main duties of the job
Assess the needs and strengths of individuals referred to the service and help them identify individual goals to achieve desired change.
Coach and motivate referred patients to identify their needs, set goals, and support them to implement and achieve their personalised health and wellbeing objectives. This will include providing patients with advice, guidance and a management plan that is personalized to their individual needs, which aims to improve mental health and well-being.
Manage and prioritise a caseload of children, young people and families, in accordance with the health and wellbeing needs of the patient, including establishing and attaining goals that are important to the patient, and providing interventions to meet them.
Ensure all interventions and coaching are designed to empower patients to be active participants in their own healthcare, empower them to manage their own health and wellbeing, live independently and involve children and young people in the co-production of activities, programmes and local services.
Work with the broader MDT to maximise the support available to patients, including working collaboratively with Surrey and Borders NHS Partnership, Surrey Children's Services, Mindworks (CAMHS), Mindworks Alliance Partners, YMCA, Childrens Community Health Providers (CFHS/FCHC) and other community-based services to provide the most effective services for children, young people and their families.

Healthy Horley PCN
Our Primary Care Network (PCN) is an NHS collaboration between 2 GP practices East Surrey Modality Medical Practice and Wayside Surgery covering Horley and surrounding areas (Outwood, Salfords and Smallfield) and has one site in Caterham. The practices have a combined population of 45,000, with approximately 30% of patients being aged between 0-25 years. There is a wide socio-economic and demographic served within this area which will provide an interesting and varied case mix.
The PCN delivers a wider range of services including a Children and Young Persons Wellbeing Service, Anticipatory Care / Frailty Hub, GP Integrated Mental Health Services, Employment Specialist Services, and a Community Health Hub (Horley Health Hub). We are proactively engaged with the Neighbourhood Health and Wellbeing Networks.
