Job summary

This is an exciting expansion to our PCN Children and Young Persons Wellbeing Service, and as such the successful post holder will have the opportunity to be involved in the development of the service, research opportunities, as well as having the responsibility for collecting user feedback and adapting the role to suit the needs of users in line with the THRIVE model and Goal Based Outcomes (GBO) Framework.

The Health and Wellbeing Coach will be part of a network-based Wellbeing Team including a Social Prescribing Link Worker to engage with children, young people and families and deliver a combination of one to one, group, and virtual interventions to improve their mental health and wellbeing.

The successful postholder will use their coaching skills to support people to develop the knowledge, skills, and confidence to manage their own health and wellbeing, whilst increasing their ability to access and use community support offers. You will work as part of a small team, undertaking initial assessments and delivering a range of evidence-based interventions, which will include holding a caseload.

Main duties of the job

Assess the needs and strengths of individuals referred to the service and help them identify individual goals to achieve desired change.

Coach and motivate referred patients to identify their needs, set goals, and support them to implement and achieve their personalised health and wellbeing objectives. This will include providing patients with advice, guidance and a management plan that is personalized to their individual needs, which aims to improve mental health and well-being.

Manage and prioritise a caseload of children, young people and families, in accordance with the health and wellbeing needs of the patient, including establishing and attaining goals that are important to the patient, and providing interventions to meet them.

Ensure all interventions and coaching are designed to empower patients to be active participants in their own healthcare, empower them to manage their own health and wellbeing, live independently and involve children and young people in the co-production of activities, programmes and local services.

Work with the broader MDT to maximise the support available to patients, including working collaboratively with Surrey and Borders NHS Partnership, Surrey Children's Services, Mindworks (CAMHS), Mindworks Alliance Partners, YMCA, Childrens Community Health Providers (CFHS/FCHC) and other community-based services to provide the most effective services for children, young people and their families.

Healthy Horley PCN

Our Primary Care Network (PCN) is an NHS collaboration between 2 GP practices East Surrey Modality Medical Practice and Wayside Surgery covering Horley and surrounding areas (Outwood, Salfords and Smallfield) and has one site in Caterham. The practices have a combined population of 45,000, with approximately 30% of patients being aged between 0-25 years. There is a wide socio-economic and demographic served within this area which will provide an interesting and varied case mix.

The PCN delivers a wider range of services including a Children and Young Persons Wellbeing Service, Anticipatory Care / Frailty Hub, GP Integrated Mental Health Services, Employment Specialist Services, and a Community Health Hub (Horley Health Hub). We are proactively engaged with the Neighbourhood Health and Wellbeing Networks.

Health and wellbeing coaches will:

Assess the needs and strengths of individuals referred to the service and help them identify individual goals to achieve desired change.

Work with the broader MDT to maximise the support available to patients, including working collaboratively with Surrey and Borders NHS Partnership, Surrey Children's Services, Mindworks (CAMHS), Mindworks Alliance Partners, YMCA, Childrens Community Health Providers (CFHS/FCHC) and other community-based services to provide the most effective services for children, young people and their families.

Maintain accurate records of patient engagement, evidence of change and celebrate progress with children and young people.

Manage and prioritise your own caseload, in accordance with the needs, priorities and support required by individuals, some with complex and multiple needs.

Have a good understanding of and utilise the THRIVE delivery model in sessions, which conceptualises the mental health and wellbeing needs of children, young people and families in terms of five different needs-based groupings: getting advice, getting help, getting more help, getting risk support, and thriving.

Apply safeguarding and child protection procedures and work closely with the safeguarding lead to ensure any issues and concerns are escalated and dealt with quickly and effectively.

Help provide written case studies as evidence of the effectiveness of individual interventions.

Work as part of a team and attend team meetings, training events and participate fully in one-to-one, peer and group supervision.

Work across the practices within the primary care network, including a combination of in person, remote, telephone and video consultation.

Patient Coaching, Care and Support:

Provide personalised coaching to patients which supports them to identify and meet their individual lifestyle, health and wellbeing goals.

Tailor health and lifestyle advice effectively to the individual.

Provide personalised support to individuals, their families and carers, and work with them holistically in managing challenging behaviours and resolving conflict.

Build trust and respect with the person, providing non-judgemental and nondiscriminatory support, respecting diversity and lifestyle choices. Work from a strength-based approach focusing on a persons assets.

Increase patient motivation to self-manage and adopt healthy behaviours.

Work with patients with lower activation scores to understand and increase their level of knowledge, skills and confidence (their Activation level) to manage their own health and wellbeing and increase their ability to access and utilise community support offers.

Be a friendly, professional and engaging source of information about health, wellbeing and prevention approaches. Support patients in shared decision-making conversations.

PCN and MDT

Promote health and wellbeing coaching, its role in self-management, addressing health inequalities and the wider determinants of health.

Provide education and specialist expertise to fellow PCN staff, ensuring they are made aware of health coaching and social prescribing services and support colleagues to improve their skills and understanding of personalised care, behavioural approaches, and ensuring consistency in the follow up of peoples goals where an MDT is involved.

Raise awareness within the PCN of shared decision making and decision support tools and supporting people in shared decision-making conversations.

As part of the PCN multi-disciplinary team (MDT), build relationships with staff in GP practices within the local PCN, attending relevant MDT meetings, giving information and feedback on health coaching.

System Responsibilities

Develop and maintain effective relationships with the community, ensuring that patients can easily move between services and community resources to access additional support.

Work in partnership with all local agencies to raise awareness of health and wellbeing coaching, and how partnership working can reduce pressure on statutory services, improve health access and outcomes and enable a holistic approach to care.

Seek regular feedback about the quality of service and impact of health coaching on referral agencies.

Alongside other members of the PCN multi-disciplinary team, work collaboratively with all local diverse partners to contribute towards supporting the local VCSE (Voluntary, Community and Social Enterprise) organisations and community groups to become sustainable and that community assets are nurtured, through sharing intelligence regarding any gaps or problems identified in local provision with commissioners and local authorities.

Develop collaborative relationships and work in partnership with health, social care, and community and voluntary sector providers and multidisciplinary teams to holistically support patients wider health and wellbeing, public health, and contributing to the reduction of health inequalities.

Data capture

Work sensitively with people, their families and carers to capture key information, enabling tracking of the impact of health coaching on their health and wellbeing, including the measures required within the PCN Contract, for example, a reduction in GP appointments for children and young people mental health concerns, reducing referrals into Mindworks, and reducing repeat attendance to the GP for mental health related issues.

Through the use of patient-reported outcome measures (PROMs) and patient-reported experience measures (PREMs); encourage people, their families and carers to provide feedback and to share their stories about the impact of social prescribing and health coaching on their lives to help healthcare providers, commissioners and other stakeholders to make informed changes to the service.

Support referral agencies to provide appropriate information about the person they are referring. Provide appropriate feedback to referral agencies about the people they referred.

Work closely within the MDT and with GP practices within the PCN to ensure that the relevant codes are captured and inputted into clinical systems, adhering to data protection legislation and data sharing agreements.

Professional development

Work with your supervisor to undertake continual personal and professional development, taking an active part in reviewing and developing the role and responsibilities.

We will offer peer to peer support and clinical supervision.

Undertake identified coaching and training as required.

Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, equality, diversity and inclusion training and health and safety.
