Park and Orchard Primary Care Network (PCN) have
an exciting opportunity to expand their Primary Care workforce by employing an Advanced
Nurse Practitioner to join their innovative and dynamic Primary Care Network,
to be based at Park Surgery.
You will be joining an enthusiastic
team of clinicians and administrators including GPs, Paramedic Practitioners,
Pharmacy Technicians, Clinical Pharmacists, Physician Associates, First Contact
Physiotherapists, Social Prescribers, Health and Wellbeing Coaches, Care
Coordinators, Practice Nurses, Healthcare Assistants and more.
Our surgery teams work closely, sharing expertise and
resources to develop new services including ambitions to soon establish a
Womens Health project and an Anticipatory Care/Frailty project. Our vision is
to continue to improve the quality of care that we provide in alignment with
the needs of our patient population and find innovative solutions in general
practice to deliver the best care we can for our patients.
Our PCN builds on the existing primary care services and
enables a greater provision of proactive, personalised and more integrated
health and social care. We are supported by practitioners in additional roles
who allow us to create bespoke multi-disciplinary teams based on the needs of
our local population. By working together with local community services, this
allows us to make support available to people where it is most needed.
Main duties of the job
The post holder is an experienced nurse who, acting within
their professional boundaries, will work as part of a multi-disciplinary team
which consists of GPs and Pharmacists. The triage team will work together to
triage all patient requests, which are received on the day, treating patients
with own clinical competency as appropriate. The post holder will see acute on
the day cases.
The post holder will act as an autonomous practitioner
working independently and in conjunction with other health care professionals
to triage, assess, diagnose and treat the conditions of patients attending
within primary care; initiating direct referrals as appropriate.
The post holder will provide expert professional advice to
patients, carers and colleagues and ensure the maintenance of clinical excellence.
Whilst
General Practice has some challenging times ahead, this role provides an
exciting and innovative opportunity for someone who wants to make a real
difference in the communities they serve. The successful candidate will be
provided with excellent support, the opportunity for development in the role,
and will be supervised by a General Practitioner.
This is a part time position of up to 30 hours per week.

Care Network (PCN) is an NHS Collaboration between 2 GP practices - Park
Surgery and Orchard Surgery covering the Horsham Locality. The practices have
a combined population of 30,000 patients. There is a wide socio-economic and
demographic served within this area which will provide an interesting and
varied case mix.

treatment/interventions and care for patients presenting with an
undifferentiated diagnosis. Clinically examine and assess patient needs from a
physiological and psychological perspective.
- Assess, diagnose, plan, implement and evaluate
interventions/treatments for patients with complex needs. Proactively identify,
diagnose and manage treatment plans for patients at risk of developing a
long-term condition.
