North
Tandridge Primary Care Network (PCN) have an exciting opportunity to expand its Primary Care Workforce
by employing a Paramedic to join their innovative and dynamic Primary Care
Network and build on the existing team as they continue to recognise the value
the additional roles bring to our practices and patients. Our aim is to provide
exemplary patient care and find innovative solutions in general practice to
deliver the best care we can for our patients. You will be joining an
enthusiastic team of clinicians and administrators. The role will contribute to
improving the quality of care of our patients.
Our PCN comprises of 5 Practices covering the North Tandridge area. The
Practices (Whyteleafe Surgery, Chaldon Road Surgery, Warlingham Green Medical
Practice, Elizabeth House Medical Practice and Caterham Valley Medical
Practice) have a combined population of 36,000 patients. There is a wide
socio-economic and demographic served within this area which will provide an
interesting varied case mix.
Within our
PCN, we have a number of innovative services that we have
implemented to identify new approaches in delivering care and improve the
health and wellbeing of our population including an Anticipatory Care Hub,
Children and Young Persons Wellbeing Service, GP Integrated Mental Health
Service, BP@Home and Enhanced Access.
Main duties of the job
This is an exciting opportunity for the successful candidate to join
the North Tandridge Primary Care Network (PCN) team in supporting a wide range
of patients of all ages. This post is part of the Additional Roles
Reimbursement Scheme for Primary Care Networks funded by NHS England.
Whilst General Practice has some challenging times ahead,
this role provides an exciting and innovative opportunity for someone who wants
to make a real difference in the communities they serve. You will work
autonomously supporting patients across the PCN, providing enhanced clinical
assessment and treatment skills to provide first point of contact for patients
presenting with undifferentiated, undiagnosed problems.
You will work closely with clinicians from the Practices
across Healthy Horley PCN and will play a vital role in increasing capacity for
all appointments including home visits.
The successful candidate will be provided with excellent
support, the opportunity for development in the role, and will be clinically
supervised by a General Practitioner.

Clinical responsibilities:
- To
provide assessment, diagnosis and treatment at first point of contact by
attending to patients according to patients needs either in the Practice or
patients homes.
- Work
with practice teams to ensure the process for referral of urgent on the day
patients is smooth and appropriate patients are referred making full use of the
practitioners skills.
- To
accurately triage and prioritise patients, including in emergency situations,
demonstrating the use of a variety of techniques to elicit the history of an
event/illness, including past medical and drug history.
- To
evaluate clinical information from examination and history taking and initiate
appropriate treatment and/or referral.
- To
undertake and interpret diagnostics and take appropriate action to manage
patient care, whilst distinguishing between abnormal and normal findings in
order to relate them to a possible diagnosis.
- To use broad clinical guidelines
(NICE etc) relating them to individual cases, providing assistance and feedback
to the organisation and others on how these should be interpreted.
- To signpost appropriately to other services in the identified
healthcare pathway.
- To refer patients to
appropriate care settings, within locally agreed healthcare pathways.
- To
advise and support members of the public on healthy lifestyles, health
promotion and disease prevention.
- To
maintain accurate and systematic documentation of patient interactions and
clinical care provided, including adverse events.
Communication:
- Ensure
via use of clinical systems, attendance at meetings and other means that are a
core part of the practice MDT supporting patients and work with the wider MDT
including staff from SCFT, SPFT, Local Authority and Voluntary Sector
organisations.
- Ensure
high quality, clear communication with patients and carers including within
challenging situations.
Professional:
- To
proactively support audit of own caseload and that of colleagues to ensure the
quality of the service is maintained an continuously improved.
- To seek out
feedback from other members of the MDT and patients to support this.
- To
undertake all necessary training and be familiar with all policies to ensure
compliance with expected procedures and ways of working.
- To
ensure practice is within good clinical governance and based on sound up-to-
date clinical evidence. Where appropriate, to identify and assess any risks
associated with patient care or to oneself and communicate these risks to the
appropriate clinical manager.
