Registered Nurse - General Practice (Asthma/Diabetes)

Job summary

The post-holder will demonstrate a high level of clinical skills developed through experience and training. You will be responsible for providing nursing services to patients, which will include treatment screening, advice and management of patient groups with complex chronic diseases within the professional competence of the post holder and in accordance with agreed protocols/patient group directions.

Main duties of the job

General

Develop, implement and embed health promotion and well-being programmes

Implement and evaluate individual treatment plans for chronic disease patients

Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health

Provide health promotion and disease prevention initiatives

Provide telephone advice to patients / care workers

Our primary working relationship is with our member practices and their Primary Care Networks (PCNs) across South Eastern Hampshire and Fareham & Gosport localities. To provide and strengthen our range of patient services SHPCA has developed successful working relationships with other local NHS healthcare providers.
