Job summary
Are you passionate about making a difference to those needing access to Community Specialist Palliative and End of Life Care? We are delighted to be able to offer the opportunity to join our Community Specialist Palliative Care Team working across the Sutton and Merton boroughs.
We work very closely with patients and those important to them, offering complex pain and symptom control, psychological and emotional support . We offer advice to GPs, District Nurses and other healthcare professionals as well as sign posting to other services where appropriate.

Main duties of the job
We are currently seeking to recruit a Band 7 Community Clinical Palliative Care Specialist and would also consider a Band 6 Development Post for the ideal candidate who demonstrates passion and commitment.

Work as a specialist palliative care resource in relation to the control of symptoms and the provision of social, economic and spiritual care for the patient and family in the community setting. To provide advice, support and education to the primary health care team where appropriate, as part of the multi-disciplinary team.
