St Raphaels Hospice are looking for a qualified HCA to work as part of our professional, friendly IPU team. To work 2 x 11 hours night shifts per week 2 in 7 20:00 to 08:00. The Ideal applicant will be able to work independently providing safe, effective care under the guidance of registered healthcare professionals, in the delivery of palliative care to patients and their families utilising protocols and procedures.

Main duties of the job

Under supervision of the RN, to carry out overall nursing care of the patient, including:

Assisting with personal hygiene, feeding, manual handling and toileting

Carrying out established bowel and catheter care

Obtaining specimens

Assisting the patient in taking medication that has been dispensed by an RN

Carrying out last offices

Managing the care of colostomies and ileostomies

Carrying out simple wound management

Monitoring and recording vital signs

Monitoring and recording blood glucose levels

Assisting in uncomplicated admissions

Performing other duties within his/her capabilities as delegated by the RN

To collect information that informs the management of the patient and report immediately any changes in the patients condition

To involve patients and families in shared decision-making, working within the principles of informed consent

To ensure that the dignity and well-being of patients are respected at all times and to treat everyone with respect

About us

St Raphaels Hospice is purpose built and set in beautiful grounds. It provides palliative care services for the 360,000 residents of Sutton and Merton. Patients are referred by their GP, Hospital Doctor or Clinical Nurse Specialist. St Raphael's has a 14-bedded in-patient unit, which has recently been refurbished, providing a beautiful working environment.

Under supervision of RN, to give general information to patients and families and practical advice to families following a patients death

Under supervision of RN, to record care delivered and significant interactions with patients and families in the patients notes, according to St Raphaels policy and procedures

To assist in the ordering, storing and stocking up of nursing stores and supplies

Education and Training

To identify own learning needs and interests, taking responsibility for own personal development

To be proactive in accessing educational and development opportunities

To utilise constructively any protected time with a facilitator, and the appraisal system, in order to reflect on practice and highlight training needs and thus to develop in the role as an HCA.

Quality Improvement

To report, without delay, any incidents, accidents and complaints to the nurse in charge

To use and maintain resources effectively and efficiently

To adhere to and keep up-to-date with health and safety, infection control and fire policies, attending mandatory training as required

To contribute to environmental and patient-focused risk assessments, taking an active role in maintaining a healthy, safe and secure working environment for everyone who is in contact with the organisation

General

To develop and maintain the necessary IT skills and experience to be able to carry out this role effectively and efficiently (seeking training and support as necessary), and to ensure, through the Hospice's appraisal, training and development processes, that all other members of the Department develop and apply the skills they require for their roles.

Other IT skills will include use of email, electronic record keeping, organisation of work diary, and tracking of waiting list and patient caseload.
