Lead Nurse - Wandsworth End of Life Care Coordination Service (WEoLCCS)

Salary: AfC Band 6

Hours: 37.5 hours per week. Including weekends and public holidays

An exciting opportunity has arisen to join the Wandsworth End of Life Care Coordination Service (WEOLCCS) as a Lead Nurse. This is a rewarding role within a dedicated team that is committed to making a meaningful difference in the lives of patients and their families.

As Lead Nurse, you will play a pivotal role in delivering high-quality, patient-centred end-of-life care across Wandsworth. You will support individuals in achieving their preferred place of care and death, help reduce unplanned or inappropriate hospital admissions and facilitate timely discharges from hospital settings.

The role demands initiative, empathy, and resilience, as it involves supporting patients and families during some of the most difficult moments in their lives. You will be expected to engage compassionately with distressed and anxious individuals, offering both clinical expertise and emotional support.

Main duties of the job

  • To be responsible for the day-to-day operational leadership and management of the Royal Trinity Carers with the support of the Clinical Hub Team Lead
  • To act as an End-of-life Care champion, building and maintaining professional relations (internally and externally) to promote the service
  • To work closely with all service delivery teams, Continuing Health Care and care agencies to ensure care is planned to meet the unique and changing needs of patients, their families, and friends
  • To provide an expert clinical resource to the team, the organisation, and external stakeholders
  • To support the Clinical Hub Team Lead and Head of service in developing and implementing service improvement

  • Understanding of the community palliative care service and the wider work of the hospice

  • Understands the importance of and confidence to participate in multidisciplinary team discussions
  • Understands the importance of recognising and responding to changes in patients condition
  • Excellent communication skills
  • Leadership experience
