It is an
exciting time at Trinity as we launch our new community services, which make up
our new clinical hub. The new services have been developed to streamline the
coordination of care for patients, address the evolving palliative and end of
life care needs of our local population and maximise the flexibility and responsiveness
we can offer.
We are looking for an experienced, enthusiastic, and
highly motivated individual to take up the post of Clinical Nurse Specialist within
our rapid response service.
This post is suited to a senior nurse with significant
experience of caring for patients with life limiting illnesses who possesses
advanced knowledge and skills in clinical assessment, symptom management and communication.
You will be a key member of the rapid response team, and the wider
multidisciplinary team working across Royal Trinity Hospices community
catchment area supporting patients with urgent and complex specialist
palliative care needs wherever they call home.
Main duties of the job
Carry out holistic assessment of patients specialist
palliative care needs and develop an initial care plan to address any urgent
needs.
Communicate and liaise with the patients usual
care team including their GP, District Nurses, existing specialist palliative
care professionals, out of hours services, reporting on the condition and needs
of the patients and providing expert advice and support to address their urgent
needs.
Establish excellent
working relationships with internal and external peers and colleagues to foster
positive and collaborative working.
Offer support, guidance,
and education to less experienced members of the rapid response team, and other
professional visitors to the team to encourage individual and team learning and
development in specialist palliative care.

The Rapid Response Service is a 24/7 service delivered across two shifts: 08:00-20:00 and 20:00-
08:00.
During those hours the Rapid Response Nurse and a Palliative Care Assistant (PCA) will respond to
the needs of patients identified as appropriate for the service across Trinitys catchment area by
providing specialist palliative nursing and end of life care.

- To support the functions of the Clinical Hub including:
- Triage and first contact
- The functions of the 24-hour specialist palliative care advice line
- Virtual Hospice Ward
- Specialist Community Nursing
- To provide expert nursing care within any existing care plans, and in accordance with the
medical care prescribed by the GP.
- To provide specialist nursing care and support to the families and carers of individual
patients in the place they call home across the Trinity catchment area, being sensitive to
their changing physical, psychosocial and spiritual needs.
- To communicate and liaise with the patients usual care team including District Nurses, other
palliative care support such as the 111 Roaming Service as well as the Clinical Hub Team
Lead, reporting on the condition and needs of the patients and providing and maintaining
clinical records, as necessary.
