Salary: AfC Band 3 £27,948 - £29,468 (inclusive of HCA) dependent on experience

Royal Trinity Hospices community catchment area spans across south east, south west and north west London integrated care systems and the team support patients with identified specialist palliative care needs wherever they call home.

We are looking for a kind and caring individual who is committed to delivering high quality person-centred care and support to patients approaching the end of life and those close to them.

The PCAs at Trinity make a vital contribution to the ongoing assessment and specialist palliative care support delivered to patients on the specialist community nursing caseloads and are the eyes and ears of our service. The support is delivered either virtually or in person. Visits are carried out independently or jointly with a registered professional, and care is always provided under the guidance and supervision of the registered professional responsible for the patient

It is an exciting time at Trinity as we begin launching our new community services, which have been designed to streamline the coordination of care for patients, address the evolving palliative and end of life care needs of our local population and maximise the flexibility and responsiveness we can offer.

Main duties of the job

Responsibilities of a Royal Trinity Hospice Palliative Care Assistant:

  • Use listening and observational skills to understand the needs of patients, families, and carers

    Communicate effectively with patients, their families, carers, using a variety of methods (e.g., face to face, telephone, or video) to build trusting professional relationships

    Support clinical and non-clinical colleagues by communicating observations of patients and other situations clearly and concisely.

    Develop excellent working relationships with the different members of the team offering community services

To support patients in their own homes to live as fully as posisble with their life-limiting condition. The PCA will be the eyes and ears for the Registered Professionals who hold responsibility for the patient's care. The PCA will use excellent communication and listening skills to enhance patients' psychological wellbeing; be adept at recognising when patients' conditions change and report this to the relevant member of the specialist Multidisciplinary team.

To communicate effectively with patients, their families, carers, using a variety of methods (e.g., face to face, telephone, or video) to build trusting professional relationships

Communicate effectively with the multidisciplinary team within the hospice and in the external community

Use listening and observational skills to understand the needs of patients, families, and carers

Develop excellent working relationships with the different members of the team offering community services

Manage conflict and challenging situations effectively and or recognises when to escalate concerns to a senior team member when needed.

To recognise a change in patients condition and communicate this effectively to the relevant clinician and to the wider interdisciplinary team as appropriate and document accurately in patients notes

Understands impact that own communication and approach may have on the wellbeing of the patient, family carer and other professionals

Contributes to multidisciplinary planning and decision-making meetings and attends decision making meetings as directed by the Community team

Ensure the values and culture of the organisation are projected in all communications
