Palliative Care Dementia Nurse Specialist

Salary:Agenda for Change Band 7 (£54,320 - £60,981 inc. HCA Supp, depending on experience pro rata)

Hours: Part time 30 hours/week (salary pro-rata)

We have an exciting opportunity to join for a PCDNS to join our friendly and supportive Specialist Community Nursing Team. We are looking for a nurse with extensive experience within the field of specialist palliative care, and a passion for caring for people with dementia.

Main duties of the job

  • Supporting people with dementia to engage in advance care planning.
  • Assessing the palliative and end of life care needs of patients with advanced dementia and their families.
  • Working closely with community health and social care agencies to co-ordinate and support the palliative care support for patients referred with dementia at the end of life.
  • Regularly teaching about the palliative care needs and management of patients with dementia at both internal and external training sessions.
