Job summary

Bury GP Federation is seeking an experienced and compassionate Frailty GP to join our innovative, integrated frailty services across Bury. This is a unique opportunity to work across hospital,community and virtual ward settings, delivering high quality, patient-centred care to some of our most vulnerable patients. You will work as part of a dynamic multidisciplinary team, supporting frail patients to remain safely at home wherever possible and providing expert clinical leadership across services.

Main duties of the job

Key Duties and Responsibilities

Act autonomously as Lead Clinical Decision Maker for patients within frailty services whenrequired

Work closely alongside Consultant Geriatricians, ACPs and the wider MDT

Provide comprehensive general medical care including:

  • History taking and examination

  • Investigation,diagnosisand treatment

  • Development and monitoring of care plans

  • Holistic assessment, including anticipatory and end of life car

  • Undertake domiciliary visits for patients with complex clinical needs

  • Maintain effective communication with:

  • Patients,familiesand carers

  • The patients registered GP whenrequired

  • Use digital technology to support remote consultation and virtual care

  • Support junior medical staff

  • Support and contribute to the wider multidisciplinary team

  • Activelyparticipatein and whereappropriate lead MDT meetings

The Frailty GP role will require working on a rota with GP colleagues providing GP input to the following services within Bury:

  • Ward 24

  • Hospital@Home - Virtual ward

  • Intermediate Care

  • Frailty SDEC - Same Day Emergency Care

The role will involve being integrated within all 4 services. The nature of cover required by each individual service will mean at times working on-site with one service whilst being available for remote advice to other services.

Frailty GPs will use their skills from General Practice to facilitate the best quality of care to frail patients and helping them to reach their full rehabilitation potential whilst also taking a holistic approach to anticipatory and End of Life Care. They will work towards the ethos of care for frail patients in their own place of residence where possible and minimise time away from that place when hospital care is unavoidable.

Qualifications

Essential

  • Primary Medical Qualification
  • Certificate of Completion of Training for General Practice (CCT) or equivalent
  • NHS Performer List
  • Full GMC Registration with a licence to practise
  • Professional Indemnity
  • Evidence of annual appraisal

Desirable

  • MRCP
  • Diploma in Geriatric Medicine
  • Diploma or apprenticeship in older people's care
  • Higher degree to Masters level

Qualities
