Job summary
We are looking for experienced Health Care Assistants that hold an NVQ Level 3 or equivalent in Health and Social Care.
Flexible - Full or Part time roles avaliable
Location - Primary Care Hubs and community settings with optional home visits
You will play a key role within a multi disciplinary team (MDT) delivering safe , effective, patient-centred anticoagulation management.
The post holder will contribute to the delivery of a comprehensive warfarin monitoring service and direct oral anticoagulent (DOAC) initiationand review service operating across primary care hubs, GP Practices, and community settings. Somestaff will also undertake home visits for patients who are unable to attend clinic appointments due to mobility or health constraints.
This flexible role involves close collaboration with clinicians and administrative colleagues to ensure continuity of care, safe prescribing and robust follow up
Main duties of the job
Job responsibilities
The post holder will contribute to the delivery of a comrehensive Warfarin monitoring and Direct Oral Anticoagulant (DOAC) initiation and Review Service operating across primary care hubs, GP Practices and community settings.
Assist
clinicians in the emergency management of patients
Provide
relevant health promotion literature for patients
Deal
with pathological specimens. Check forms are correctly completed. Arrange for
specimens to be sent to the path lab and record details of what was sent
Follow
infection control standards
Contribute
new ideas to improve patient care
Attend
educational training courses that relate to your job and the objectives of the federation,
when requested
Recording
of ECGs, 24-hour blood pressure, 24-hour ECGs
Venepuncture desirable
Vaccinations
Adult vacations as part of the programme
Wound
Management - Change dressings and removal of stitches

Qualifications
Essential: Experienced HCA with NVQ Level 3 or equivalent in Health and Social care. Additional Anti coagulation qualification (eg BSH competency certificate, INRstar accreditation) desirable.

- Liaise with secondary care anticoagulation clinics, ensuring smooth transitions for newly discharged patients or complex cases.
