Job summary

Waltham Forest Federated GP Network is looking to recruit an Independent Prescriber Clinical Pharmacist who will have a key role in supporting the delivery of the new COVID-19 Therapeutic service across North East London.

The purpose of the service will be to provide a COVID-19 therapeutics service for non-hospitalised patients who will benefit from them the most. The service is required to provide timely access to medication by way of a clinical triage process. This service aims and intends to improve the timely access and efficiency of the supply of COVID-19 therapeutics for the treatment of COVID-19 to eligible patients and provide a local pathway (acute or primary care) ensuring routine access to COVID-19 treatment for individuals at the highest risk of hospitalisation.

Main duties of the job

To form part of the clinical team that delivers the CMDU Therapeutics Service

To work with WF GP Federation and system stakeholders to help deliver an efficient, safe and effective service

To work with the clinical and administrative team to provide CMDU functionality

Job description

Job responsibilities

With the support of the organisation, the post holder will be responsible for:

  • Leading, developing, co-ordinating and implementing the delivery of medicines safety agenda within the C19 Therapeutic service.
  • Ensure the provision of a high-quality clinical service which meets the needs of patients within the service
  • The postholder will perform remote medication reviews of patients following a positive PCR or lateral flow test and undergo clinical assessment, triage and prescribing via local arrangements in place.
  • Clinical triage patients over the phone, considering:

Timing of symptom onset

Assessment of severity of symptoms to confirm the patient does not require hospitalisation

Vaccination status (to assess for risk of disease progression)

Past medical history

Drug and allergy history, including any previous history of anaphylaxis

Decide on the correct treatment for the patient

Document clinical triage telephone call and consent

Prescribe the suitable form of treatment

Complete discharge letter

  • Analyse, interpret and present medicines data to highlight issues and risks to support decision-making.
  • Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components
  • Liaises with other stakeholders as needed for the collective benefit of patients Including but not limited to

1. Patients and their representatives

2. GP, nurses and other practice staff

3. Locality / GP prescribing lead

4. Locality managers

  • Completion of reporting requirements for quality and assurance supported by the organisations data teams.

  • Making effective use of training to update knowledge and skills
  • Actively reporting health and safety hazards and infection hazards immediately when recognised
