Description of Roles
Waltham Forest Federated GP Network are supporting the Primary Care Networks across the borough to recruit Advanced Nurse Practitioners.
They will provide nursing assistance to GPs within the PCN and work collaboratively with the clinical and administrative teams to meet the needs of patients while adhering to the relevant policies and procedures and providing nurse leadership as required.
Patients enquiries must be handled courteously, efficiently and with empathy. Record keeping, adherence to protocols and procedures and the sharing of information within the team should be undertaken efficiently and effectively. It is essential that a strict code of confidentiality is always observed.
It is important that all members of staff are prepared to take on additional duties or relinquish existing duties in order to maintain the efficient running of the PCNs practices.
Primary duties and areas of responsibilities -
Work autonomously and be accountable for his/her own professional actions.
Undertake telephone triage work and telephone advice calls.
Undertake clinical nursing practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to patients.
Assess diagnoses and treat patients services with a range of acute, non-acute and chronic medical conditions, including the following:
Respiratory:
- Exacerbations of Asthma or COPD;
- Cold;
- Sore throat;
- Chesty cough;
- Chest infection;
- Wheeze;
- Shortness of breath (talking in full sentences).
Ear, Nose & Throat:
- Ear discharge & or pain;
- Blocked ears;
- Vertigo/room spinning/dizziness;
- Sinus pain;
- Hay fever (eye itch/running nose);
- Sore throat/tonsillitis.
Gastric Conditions:
- Heartburn;
- Indigestion;
- Lower abdominal pain;
- Piles;
- Constipation;
- Diarrhoea and vomiting.
Skin Conditions:
- Shingles painful blister-type rash;
- Cellulitis skin infection (red swollen);
- Infected insect bites;
- Infected cysts/boils;
- Painful big toe (gout);
- Cold sores;
- Fungal foot infections (athletes);
- Ringworm;
- Ingrown hair/toenail.
Genital Urinary Conditions:
- UTI;
- Thrush;
- Vaginal discharge;
- Contraception;
- Menstrual Bleeding.
MSK:
- Traumatic or non-traumatic symptoms within joints where the patient is able to weigh bare or still use the joint.
Cardiology:
- Refers patients to other medical specialities for assessment if deemed appropriate.
- Interprets a range of diagnostic tests and routine clinical procedures.
Delivering a quality service -
- Recognise and work within own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC)
- Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures
- Prioritise, organise and manage your own workload in a manner that maintains and promotes quality
- Work within policies relating to domestic violence, vulnerable adults, substance abuse and addictive behaviour and refer as appropriate
Communication -
Provides and receives highly complex, sensitive or contentious information and frequently deals with situations which are hostile or emotive.
Uses developed communication, negotiation, and conflict management skills which require empathy, reassurance and persuasive skills where there can be significant barriers to acceptance which need to be overcome.
Utilise Interpreting Support Services when appropriate.
Manage and use information -
Makes effective use of the clinical system by ensuring the consistent and accurate entering of data.
- Use technology as an aid to management in planning, implementation and monitoring presenting and communicating information.
PERSON SPECIFICATION
EDUCATION AND QUALIFICATIONS - Essential
- Qualifications/Certificates Qualified Nurse
- Independent Nurse Prescribing Qualification
- A demonstrable commitment to professional development.
PREVIOUS EXPERIENCE - Essential
- Experience in working in accordance with the UKCC Code of Professional Conduct and The Scope of
- Professional Practice.
- Experience with EMIS Web clinical system and Microsoft Office software.
- Experience in dealing with the public/patients.
- Experience in General Practice (2 years minimum).
- Experience as an ANP (2 years minimum).
SKILL, KNOWLEDGE AND ABILITY - Essential
- Competent in nursing duties required for the post.
- Record of achieving deliverable objectives within the timeframe and finding innovative solutions to complex problems.
- Excellent communication oral and written communication skills with the ability to explain complicated concepts.
- Ability to demonstrate professional and clinical knowledge commensurate with the post.
- Well organised and able to plan and prioritise, working well under pressure and tight deadlines.
Desirable Skill, Knowledge and Ability
- Excellent IT skills
- Experience in developing operational procedures in a clinical setting.
