Job summary
An exciting opportunity has arisen
for recruitment of a PCN Dietitian to work alongside the existing Senior dietitian
within Wythenshawe PCN. Primary care networks (PCNS) think about the wider health of their
population, taking a proactive approach to managing population health.
The role will principally be
delivering in-person clinics (both 1:1 and small groups) within GP practices
and other community settings. With the existing dietitian, you will collaborate
in reviewing, adapting and developing the service to reduce the barriers to
access and engagement. Wythenshawe PCN encourages using evidence-based practice
in innovative and creative ways to improve health outcomes.
Wythenshawe PCN is situated in South Manchester and consists of 7 GP practices and serves 60,000 patients.
The practices are Benchill Medical Practice, Bowland Medical Practice, Cornishway Medical Practice, Maples Medical Centre, Peel Hall Medical Centre and RK Medical Practice.
This is a part time role for 2 days (15 hours).
Practices are open from 8.30-6.30 so working hours can be flexible.
Main duties of the job
The purpose of this flexible role is to
deliver a dietetic service offer which meets the unique and diverse needs of
service-users from the local community with an expectation it will continually
evolve with changing needs and priorities of patients.
This role is one of an experienced
practitioner who can work autonomously, but actively engages with a wider team
of professionals, both in primary care and relevant community services.
Re-establish and run clinical sessions for 1:1
patient care and small group workshops.
Deliver services in a variety of settings,
including but not limited to community clinics, GP surgeries, patients
own-homes, community venues, and group settings.
Promote the role of the dietitian
within the GP surgeries and wider network by attending meetings and events to
network with other community services and neighbourhood teams.

We are a group of 7 GP Practices in Wythenshawe, South Manchester who have come together to form a Primary Care Network covering a total population of approximately 60,000 patients.
We work together in a collaborative way to develop and deliver network-based services that respond to the needs of the local population.

Qualifications
Essential
- Undergraduate or postgraduate HCPC approved dietetic programme
- Health and Care Professions Council (HCPC) registration
- Can demonstrate working at Level 7 (masters level)
- 3 years post-graduate experience in a generalist dietetic role
Desirable
- Completed or working towards Supplementary Prescribing rights by completing a HCPC approved non-medical prescribing qualification
Experience
Essential
- Strong knowledge base and experience in weight management, frailty, gastro and Type 2 diabetes.
- Confidence in treating and managing a wide range of patient conditions.
- Able to carry out assessment, interpretation, individual care planning, motivation, monitoring and evaluation of highly specialised dietetic treatment
- Working knowledge of Microsoft and GP IT systems
- Experience of contributing to and service delivery/evolving evidence led practice and delivery in a changing environment
- Proactively engage in CPD and skills development.
Desirable
- Experience of working with a diverse range of stakeholders
- Cognitive behavioural approaches and skills
- Understanding of health inequalities and impact of poverty on health
- Knowledge of EMIS patient record system
