It
is well established that there is ongoing investment in expanding the access to
community based Mental Health (MH) services for children and young people
(CYP) under government drivers like
the NHS Long Term Plan. In response to these drivers Hammersmith, Fulham,
Ealing & Hounslow (HFEH) Mind remains in the forefront of service provision
and is committed to working in partnership with the NHS and other MH providers
to ensure more CYP can accessMH treatment and specialist
support.
At
HFEH Mind we ensure we
understand the needs of our community so that we can tailor our
response accordingly by providing appropriate services like talking therapies, peer support, advocacy,
crisis care, employment, and housing support. In response to this and government
plans for expansion and provision an exciting Crisis
Café referred to as Circle is being developed by HFEH Mind as part of the newly
established Complex Needs Hub.
The purpose of the out of hour mental health is
to help alleviate mental health and wellbeing during heightened arousal or
crisis through supportive therapeutic provision within the local community
facilitated by the Circle team. As the
out of hours Mental Health Practitioner, you will be a key link between
the Youth services core day services and Crisis Prevention services working
with CYPs between the ages of 5 18 (up to 25 for non crisis services)
years old who present or referred to this crisis service.
Main duties of the job
This Circle
SMHP role would suit a Health Care Professional (HCP) who can be a qualified
Nurse (RMN, RLDN)/School Nurse, Social Worker (SW), Occupational Therapist (OT)
Children or a Young Persons Wellbeing
Practitioner (CWP), Educational Mental Health
Practitioner (EMHP)with
a min 2 years post qualification and some formal training in CBT interventions,
experience of working within a multi-disciplinary CYP MH team with experience in
managing CYPs presenting in a MH crisis/acute MH presentations with an
interest in developing their crisis response therapeutic practice and working creatively
to provide effective evidence based de-escalation in non-clinical settings.
The post holder will mainly deliver short-term
support for CYPs in crisis and will carry out screenings, risk assessments,
safety plans and interventions using evidence based therapeutic treatment
modalities on- site. You will also work to ensure a seamless onward transition to
self-management, relevant specialist Community Mental Health services, Social
Care or other relevant partner agencies where required.

- Be

an outcomes framework, evaluation, and practice in relation to service users
mental health to include assessing risk and protective factors using
individual, family, learning environment, and community domains
to ensure we offer a highly responsive and adaptable service.
- Support
CYPs experiencing some form of Mental Health crisis that may find a less
stimulating and highly supportive environment quite beneficial. You will assist
in the delivery of evidence-based 1:1 and group interventions informed by a
variety of treatment modalities like CBT, DBT and Systemic practice in the HUB.
- Work
with people from different cultural backgrounds using interpreters when
necessary and as such should be committed to inclusivity and managing diversity
as a typical part of your work ethic. You will also work closely with
colleagues and partners in the Local Authority, CAMHS, education settings and
other relevant mental health providers and should be committed to developing
and fostering professional relationships.
- Coordinate an active allocation of children
and adolescents presenting in crisis with mild moderate emotional and mental
health difficulties and will deliver evidence-based individual and group interventions
in the Circle. This will include the
delivery and coordination of interventions directed at emotional regulation for
CYPs in acute crisis; co-produce individualised crisis plans and facilitate
relevant psychoeducation with CYPs and parents/carers. These therapeutic
interventions will have a psychosocial scope, be aimed at stress reduction,
have a low stimulus therapeutic approach to mental health and be typically
facilitated in the Circle.
