Reporting to the Head of Hospice at Home the Clinical Nurse
Specialist plays a crucial role in delivering high-quality palliative and
end-of-life care to patients in their own homes.
With a focus on holistic and personcentered care, the
Clinical Nurse Specialist provides expert clinical support, assessment, and
coordination of care, working collaboratively with patients, families, and
multidisciplinary teams to ensure the best possible outcomes.
Main duties of the job
- Provide expert clinical assessment, monitoring, and management of complex palliative care needs for patients receiving care in their own homes.
- Develop and implement individualised care plans in collaboration with patients, families, and other healthcare professionals, ensuring holistic support and symptom management.
- Offer emotional support, education, and guidance to patients and their families, empowering them to make informed decisions about their care and treatment preferences.
- Act as a key point of contact for patients and families, providing ongoing communication, reassurance, and advocacy throughout their care journey.
- Coordinate care delivery with members of the Hospice at Home interdisciplinary team, including healthcare assistants, social workers, therapists, and volunteers, ensuring seamless continuity of care.
- Collaborate with community healthcare providers, primary care teams, and specialist services to optimize support and resources for patients and families in the home setting.

- 24-hour, 14-bed Inpatient Service

Job description
Job responsibilities
