Job summary

  • This is an exciting opportunity to join our Hospice Community Service as a Senior Nurse.

  • Your role will be to provide advice around palliative and end of life care over the telephone and managing symptoms, providing holistic care to patients and their families out in the community. You will work collaboratively and support our staff nurses on the inpatient unit when needed by providing senior clinical nursing advice and nursing care where capacity is available. Support, education, and training will be provided to care and nursing homes operationally and as part of a formal programme being developed.

  • The service will work cohesively with the overnight service with an expectation to assist with caseload management and booking visits/follow up. The second phase of the service will involve more cohesion with the inpatient unit providing prescribing support and end of life admissions overnight.

Main duties of the job

  • To act within your professional boundaries and capabilities, providing care for the presenting patient from understanding and listening to the patients needs, initiating the best course of treatment, and evaluating that episode of care.
  • Organise and deliver the planned and responsive community service ensuring 24/7 continuity of patient care and integration with other Hospice and wider health economy services.
  • Demonstrate safe, clinical decision-making and care, including assessment and diagnostic skills for palliative and end of life patients who are referred into Rotherham Hospice Services.
  • Leads the delivery of the Hospice advice and support line. With assistance of support tools devised by Hospice medics and clinical nurse specialists.
  • Deliver operational supervision, PDRs and appraisals for Health Care Support Workers.
  • Undertake visits to verify expected deaths, following hospice protocol.
  • Providing emotional and spiritual support for the families.
  • Support patients and their families with advanced care planning. Explanation of the RESPECT process and completion of the documentation.
  • Prescribing for both community patients and those on the inpatient unit if required as 2nd phase of the developing service
  • Organise admissions to the inpatient unit as part of the developing service.
  • Provide education and support to the nursing and care homes in conjunction with the named care home nurse liaison service.

We began offering Day Hospice services in 1994, and have grown over the years to now provide a 14 bed Inpatient Unit alongside our Hospice Community Team who provide care in the community. Our dedicated team of staff and volunteers work together to provide palliative care that incorporates psychological, physical, social and spiritual support to help patients and their families achieve the very best quality of life, both at home and in the Hospice.
