Job summary
The job holder will work under the supervision of a Community Clinical Nurse Specialist or Paramedic to promote, plan and provide high standards of palliative care. You will provide clinical expertise to support and facilitate individualised patient centred care to patients and carers with a life limiting illness.
You will work in collaboration with the MDT and facilitate and support the implementation of evidence-based practice and contribute to the on-going development of the service.
You will participate in clinical and educational activities related to developing specialist palliative care in the community and be able to participate in a seven day per week operational service.
Main duties of the job
You will be expected, under supervision, to deliver high standards of personalised care and identify the needs of patients through holistic assessment. You will manage the role of duty nurse working with support from the triage nurse specialist.
You will develop skills to enable you to hold a small caseload, under supervision. You will discuss all patient visits with your mentor or agreed deputy. You will assess, plan and implement and evaluate all aspects of patient care. You will provide symptom control, social, emotional and spiritual care for the patient, family and carers.
You will be able to take responsibility for your own continuing professional development and performance, including identifying your own development needs. You will follow the outlined progression pathway towards a Band 7 Clinical Nurse Specialist with the support from the Community Services Manager and Team Leads.

We are a patient centred charity that delivers palliative and end of life care to thousands of people each year with advanced life-limiting illnesses who live in Northwest Surrey. The Hospice also provides support to their carers and families in patient homes, in care or nursing homes, or through its Inpatient Unit in the Hospice, as well as through the Bradbury Wellbeing Centre. This role is an essential component in supporting and complementing the Hospice Community Services Team, ensuring that the highest standards of care are implemented and that services meet statutory regulations and national standards.
