Band 5 Registered Nurse - Hospice at Home (Community based)

Job summary
You will support patients with palliative or end of life prognosis and their families / carers to achieve their preferred place of care. You will work under the supervision of the Clinical Nurse Specialists to develop knowledge and expertise in palliative care whilst providing a timely service. You will assist the Clinical Nurse Specialists to manage a caseload of patients supporting with telephone calls, home visits, bereavement support and symptom management. You will provide clinical leadership to junior members of the team with support, teaching and clinical guidance.
Main duties of the job
The RGN will be expected, under supervision, to deliver high standards of personalised care and identify needs of the patients through holistic assessment.
The RGN will use their experience and knowledge of palliative care to deliver appropriate symptom control, clinical and personal care, and medication administration.
They will work collaboratively with the Community Specialist Palliative Care Team (CSPCT), GPs and district nursing teams.
They will monitor patients on an ongoing basis, to evaluate outcomes and adjust care plans to ensure optimum co-ordination and delivery of care.
The RGN will take responsibility for their own continuing professional development and performance, including identifying own development needs.

We support patients, carers and families at home, in care or nursing homes, or at our In-Patient Unit in the Hospice, and via our Bradbury Wellbeing Centre. This role is vital in the Hospice at Home Team within Community Services, ensuring high quality care in the community.

- RN educated to Diploma or Degree level
