Job summary

To support Patients with palliative or end of life prognosis and their families/carers to achieve their preferred place of care.

To work under the supervision of Registered General Nurses, Clinical Nurse/Paramedic Specialists to develop knowledge and expertise in Palliative care whilst providing a timely service.

Assisting Registered General Nurses to manage a caseload of patients supporting with personal care visits, telephone calls, bereavement support and symptom management.

Main duties of the job

The HCA will be expected, under supervision, to deliver high standard of personalised care and identify needs of the patients through holistic assessment.

Use their experience and knowledge of palliative care to deliver appropriate symptom control, clinical and personal care.

To work collaboratively with the Community Specialist Palliative Care Team (CSPCT), GPs and district nursing teams.

To provide supportive care to patients, their families, and carers.

To monitor patients on a daily basis, to evaluate outcomes and adjust care plans to ensure optimum co-ordination and delivery of care.

Assist the community team with visiting and telephone contact to patients /families as appropriate.

Advise on and facilitate bereavement support for bereaved relatives.

Undertake bereavement visits as appropriate.

To develop and maintain effective relationships with all members of the multi-disciplinary team to ensure collaborative working for the benefit of patient care.

To support the development and implementation of care pathways and patient information.

To maintain accurate and up to date patient records.

To provide a high standard of personalised and on-going care for bereaved families.

To support and care for work colleagues whilst working in an emotionally stressful environment.

To ensure that the dignity, safety, and confidentiality of all patients is respected and that all patients receive the highest standards of care.

About us

We are a patient centred charity that delivers palliative and end of life care to thousands of people each year with advanced life-limiting illnesses who live in Northwest Surrey. The Hospice also provides support to their carers and families in patient homes, in care or nursing homes, or through its In-Patient Unit in the Hospice, as well as though the Bradbury Wellbeing Centre. This role is an essential component in supporting and complementing the Hospice training and development programmes ensuring that the highest standards of care are implemented and that services meet statutory regulations and national standards.
