Work within our practices in the Wellingborough neighbourhood to deliver high-quality, same-day care to our patients.

Were looking for an experienced Advanced Nurse Practitioner to join our Neighbourhood Access Service, part of the innovative New Models Programme. You'll work autonomously and as part of a multidisciplinary team within primary care to provide expert assessment, diagnosis, treatment, and evaluation for patients presenting with minor illness, acute conditions, and common general practice presentations, including women's health.

Role Summary

Overview of Responsibilities

Clinical

  • Provide clinical care to our patients. This will include reducing the demand on the GP time by the assessment and manage of patients and in the community by means of telephone triage and clinic appointments.
  • Use advanced skills and expert knowledge to assess the physical and psycho social needs of patients with common primary care conditions and minor illness, instigating and providing in partnership with other care providers, therapeutic treatments based on best available evidence in order to improve health outcomes.
  • Use advanced skills and expert knowledge to make comprehensive and focussed assessments, independently and proactively managing care of the patients.
  • Promote patient independence and self-care where appropriate.
  • Undertake the role of independent non-medical prescriber in the treatment and care of patients
  • Review pathology results to ensure appropriate treatment and care
  • Provide appropriate non clinical support to the GPs as per the practice needs.
  • Support and lead the practice for the delivery of the Quality and Outcomes Framework and NHS England/Nene CCG/Federation Contracts.

Regulatory

  • Respond to complaints and clinical incidents/significant events as appropriate to ensure robust systems are in place for improving practice through joint learning.
  • Undertake at least one clinical audit a year. Evaluate data and present findings to the Practice Clinical team. Implement changes where appropriate.
  • Adhere to the Practice Policies and Procedures including Infection Control, Health and Safety
  • All staff must be familiar with, actively promote and work within the spirit of the practices Equal Opportunities Policy at all times.
  • All staff who have access to personal data in relation to patients or staff will be aware of their responsibilities under the Data Protection Act 1998 and will abide by the eight principles of that Act.Any breach of the Act could result in disciplinary action being taken and criminal charges being brought against the individual.
