Job summary
We have a vacancy within our Community Services, in the shape of the role of Community Programmes Coordinator for our Active Wellbeing Service.
Commissioned by SWL ICB and delivered in partnership Enable, the Coordinator Post will focus on the Wandsworth element of the project.
Through a sub contracted relationship with Enable, working day to day within their wider team and operational management, whilst supported and employed by BWW Mind.
Focused on 1:1 physical activity support programme for SMI patients developed by Enable, with aims to reduce cardiovascular risk in patients with severe mental health conditions, increase community engagement and focus on independent health and wellbeing.
You will work with patients with severe mental illness (SMI) Schizophrenia, Bipolar, Psychosis, Severe Clinical Depression, coordinating their care through the service, through referral, assessment and ongoing support and outcome measurement as they work with an instructor.
Main duties of the job
The service approach includes
- Provide 10 weeks of 1:1 exercise sessions, bespoke to overcoming their barriers and reaching goals
- A team of 10 instructors
- Flexible and collaborative approach
- Provide 3, 6 and 12 month follow ups for ongoing support and encouraging sustainable progress using exit routes

- Nutrition (Internal)
- Diabetes Awareness and Prevention (Xyla)
