We have an exciting
opportunity to work with Wandsworth GP practices and primary care networks.
The role will take overall responsibility for the delivery of
several digital projects at scale and will requirecomprehensive stakeholder engagement and integration across Wandsworth health
and social care.
The postholder will

include but not limited to specification writing, project mobilisation plans
and oversight of delivery

Main duties of the job
Work with Practice, PCN and IT and network managers to develop digital maturity plans for practices and PCNs.
Work with Primary care and IT and network managers to identify areas of growth and service development for IT/digital projects.
Work with SWL ICB digital champions and managers on digital maturity plans and implementation of digital aspects of modern general practice.
Develop and identify digital solutions that could be implemented at scale across the 9 PCNs and practices.
Understanding of quality improvement; process development and improvement; automation and facilitate the implementation of agreed solutions at scale across practices and PCNs.
Horizon scanning identify digital solutions that could be implemented at scale across the 9 PCNs and practices i.e. GP Automation.
Understanding and application of new tech solutions that are focussed on practice/PCN needs i.e. workflow optimisation, and other areas of increasing productivity.
Attend digital meeting as required and represent the Wandsworth GP Federation in the capacity of digital lead.
Ensure there is continuity and handover of initiatives successfully implemented at the conclusion of the contract

practice-owned community interest company established to support and develop
local general practice and to provide quality healthcare for all the people of

The Wandsworth GP Federation is the lead provider
for the Multi-specialty Community Provider (MCP) in Wandsworth, integrating
primary and community health & care services in the borough.

collaborative working relationships with the 9 local Wandsworth Primary Care
Networks, operating as Primary Care Wandsworth, to ensure a strong voice and
influence for primary care in the further development of place arrangements as
part of the South-West London Integrated Care System (ICS). The Wandsworth GP
Federation is also a founder member of the South-West London Primary Care
Provider Alliance, a strategic collaboration of the 6 GP Federations within the
Southwest London ICS area.
