Job summary
To work as a core member of the nursing and
rehabilitation teams.
Assessing patients personal aims and interests to
plan appropriate therapeutic and leisure activities both for patients
accessing day services and on request for patients on the in-patient unit
and in the rehabilitation department.
To assist with nursing care duties for patients as
necessary.
To lead group activities such as gentle exercise or
relaxation programmes under the guidance of senior nursing or
rehabilitation team members.
Under the guidance of the Wellbeing Lead, liaise
with external providers as necessary.
Work in conjunction with Wellbeing Lead and
Volunteer Manager to help coordinate volunteer activities as required.
Previous
experience is Palliative Care is desirable but not essential as full training
will be given.
Main duties of the job
Ensure fundamental care needs are met
appropriately ensure patients and family members, as appropriate, receive
full information about their condition, treatment and care options Respect
working as a member of the multi-disciplinary team to treat all patients at all
times with regard to their right to dignity and privacy
Safety working as a member of the team to deliver
care that is safe
Promoting independence deliver care in a manner
which promotes patient independence
Rest and activity ensure the patients needs for
periods of rest and activity are assessed with care plans
Maintaining Comfort ensure patient is
as pain free as possible and comfortable
Nutrition
ensure patients are offered food that meets their nutritional and personal
requirements
Well-being
Working closely with the Wellbeing Lead and day
services team to plan a core programme of annual activities as a base provision
within the wellbeing unit
Working with the team assess interests of
individual patients and as appropriate identify appropriate therapeutic
activities either one to one or group to meet defined goals
To lead
specified group activities, relaxation/gentle exercise supported by other staff
members or volunteers

Nightingale House is an independent charity committed to delivering specialist palliative care services,
completely free-of-charge, to patients and their families across a wide area
stretching from Wrexham, Flintshire and East Denbighshire to Barmouth and the
border towns including Oswestry and Whitchurch.
Services include a 12 bed inpatient ward, wellbeing unit, an
outpatient clinic, occupational therapy, complementary therapies, physiotherapy
including a hydrotherapy pool and an ambulance service. A range of bereavement
support services are offered including a specialist service for children and
young adults.

provide direct nursing care to patients working independently when appropriate
or in partnership with registered nurses when patient needs demand this.
- To work together with the multi-disciplinary
team to maintain a holistic approach to patient care reporting concerns as
necessary to team members.

as needed, reporting any difficulties to the registered nursing
staff.
- To observe and report accurately on
patients general condition ensuring that all information is communicated
by taking part in discussion and recording relevant information in patients
records to comply with legal and professional requirements.
- To maintain a high standard of nursing

environment including knowledge of necessary policies (fire and health and
safety policy) and ensure equipment used is in good working order. Report accidents to Senior Nurse.
