Job summary

We are seeking an experienced and motivated Primary Care Paramedic to join Lyme Valley Practice within Newcastle Central PCN. This is a dynamic, autonomous role focused on urgent home visiting, care home support and same-day acute assessment. You will play a key role in supporting GP capacity, improving access and delivering high-quality patient-centred care in the community. We are looking for a confident clinician with strong assessment and decision-making skills who thrives in a fast-paced environment and values teamwork. Main duties of the job

You will undertake urgent and routine home visits, assess and manage acute presentations, support same-day triage and contribute to multidisciplinary care planning. The role requires excellent clinical assessment skills, sound judgement and the ability to manage undifferentiated illness autonomously. You will work closely with GPs and the wider team to ensure patients receive safe, timely and coordinated care. We are looking for a registered paramedic with strong communication skills, resilience under pressure and a compassionate, patient-centred approach. Experience in urgent or primary care settings is desirable.

Hours: Full Time 37.5 hours per week

Accountable to: Practice Manager / Clinical Lead GP

Location: Lyme Valley Practice, ST5 3TF (with home visiting across locality)

Job Purpose

The Primary Care Paramedic will work as an autonomous clinician within Lyme Valley Practice to deliver high-quality acute assessment, triage, and home visiting services.

The role has a particular focus on:

Urgent and routine home visiting

Care home clinical support

Same-day assessment and triage

Management of undifferentiated presentations

Supporting frail and vulnerable patients in the community

The post holder will support the reduction of GP workload, improve access to urgent care, and help prevent avoidable hospital admissions through proactive and responsive clinical intervention.

The post holder will:

Be registered with the Health & Care Professions Council (HCPC)

Work within HCPC Standards of Conduct, Performance and Ethics

Practice within scope of competence and experience

Maintain professional registration and ongoing CPD

Work in accordance with local and national clinical guidelines

Key Responsibilities

A. Clinical Duties

The Primary Care Paramedic will:

Undertake urgent and planned home visits, including residential and nursing homes

Assess, diagnose and manage patients presenting with acute illness

Provide same-day triage and urgent appointment support

Manage undifferentiated presentations using advanced clinical assessment skills

Order and interpret diagnostic investigations

Develop and implement management plans

Prescribe safely and appropriately in line with local policies (if qualified)

Refer to secondary care or community services as appropriate

Provide safety-netting and follow-up arrangements

Support proactive management of frail and complex patients

B. Acute Practice Support

Support same-day access demand within the surgery

Work alongside GPs to provide first-contact assessment

Contribute to minor illness and urgent care clinics

Assist in managing on-the-day clinical workload

C. Multidisciplinary Working

Work collaboratively with GPs, nurses, pharmacists, and wider PCN staff

Participate in MDT meetings where required

Contribute to coordinated care planning

Support communication with community services

D. Service Development & Quality

Contribute to the development of the home visiting model

Participate in clinical audit and quality improvement

Support delivery of enhanced services

Contribute to achieving practice and PCN quality targets

Engage in service redesign initiatives where appropriate

E. Governance & Compliance

Maintain accurate, contemporaneous records on Systm1

Adhere to safeguarding, confidentiality and information governance policies
