This is a new role for the team at Well Street Medical Centre. We are looking for a passionate and highly skilled Advanced Care Practitioner to join our forward-thinking GP practice. As an ACP, you will play a key role in delivering exceptional patient care - assessing, diagnosing, treating, and managing a wide range of conditions with autonomy and clinical expertise.

You will work closely with our GPs, nurses, and wider healthcare team, contributing to innovative, patient-centred services that truly make a difference in the community. To succeed, you will need advanced clinical skills, sound decision-making, and a commitment to continuous learning.

Main duties of the job

As an Advanced Care Practitioner in our team, you will provide high-quality clinical care across a diverse patient caseload. You will assess, diagnose, and manage acute and long-term conditions, carry out examinations and investigations, prescribe medications, and develop treatment plans. You will support health promotion, disease prevention, and patient education, working both independently and as part of our multidisciplinary team.

We are looking for a confident clinician with advanced assessment and prescribing skills, excellent communication, and the ability to make sound clinical decisions under pressure. You will need a patient-centred approach, adaptability, and a drive for continuous professional development. About us

As one of seven practices in the Moorlands Rural PCN, we care for 7,500 patients in a semi-rural town. Our friendly, multi-skilled team includes GPs, nurses, pharmacists, dispensers, admin, and management staff, supported by our ARRS colleagues including a physiotherapist, mental health practitioners, pharmacy team, care coordinators, and social prescribers.

We deliver high-quality, person-centred care, including regular nursing home visits, MDTs with our local community partners and provide dispensing services for our community.

The Advanced Clinical Practitioner (ACP) is an experienced, registered healthcare professional who has been assessed as competent in advanced clinical practice through education at Masters level (or equivalent) and mapped against the national Multi-professional Framework for Advanced Clinical Practice.

The ACP works autonomously, with the authority to assess, diagnose, treat, prescribe, and manage patients presenting with complex needs, uncertainty, and risk, within the limits of their professional competence. They apply advanced clinical reasoning to support safe decision-making, while ensuring appropriate escalation or referral when necessary.

The ACP provides timely, patient-centred care and supports avoidance of unnecessary GP referral, contributing to improved access, continuity, and outcomes for patients. In primary care, the ACP builds long-term relationships with patients, carers, and families, working in partnership to achieve optimum health.

The ACP role is underpinned by the four pillars of advanced practice (clinical practice, leadership and management, education, and research). Protected time and structured job planning will ensure the ACP contributes to each pillar, supporting personal development, service transformation, and organisational objectives.

  • Support the practice/PCN team in managing clinical emergencies and safeguarding matters, in accordance with policy.
  • Practise within their code of professional conduct and scope of practice, being responsible and accountable for their decisions, actions, and omissions.
  • Demonstrate a critical understanding of their broadened responsibility and autonomy, including working with complexity, uncertainty, and incomplete information.
