Job summary

Newcastle South Primary Care Network (PCN) comprises of 6 separate member GP Practices working together to provide patient care to a population of just over 43,000.

The PCN continues to deliver innovative services to meet the needs of our local population. As part of our on-going development an exciting opportunity has arisen for a Pharmacy Technician to join our team.

Your role will be integral when it comes to delivering the best patient care, and the successful candidate will possess strong administrative skills with the ability to multi-task and have a keen eye for detail. You will need the ability to communicate well with patients as you will often be the first point of contact for the well-being services. You must be able to listen to patients and demonstrate empathy putting them at ease.

Main duties of the job

Take an approach that is non-judgemental, based on strong communication and negotiation skills, while considering the whole person when addressing existing issues

Support patients to establish and attain goals that are important to the patient and use health coaching and motivational techniques to support patients in managing their wellbeing. Working with multiple members of the PCN team to coordinate the booking of appointments to ensure a good patient experience

Assist with the identification, tracking and recalling of patients for reviews, follow-ups and wellbeing checks, and keep a register of the teams workload, liaising with members across all practices within the PCN.

Maintain accurate records and contribute to data collection for service evaluation

Generate reports to help analyse appointment data to understand capacity and demand.

About us

Newcastle South PCN is made up of 6 member GP Practices with a combined patient population of circa 43,000. The practices are from both urban and rural areas, with significantly differing patient demographics: -

  • Ashley Surgery

We are a dynamic and forward thinking PCN team comprising of Clinical Pharmacists, Pharmacy Technicians, Care Coordinators, Social Prescribing Link Workers, First Contact Physiotherapy and Mental Health Practitioners, Health and Wellbeing Coach and GP.

We work collaboratively with our local GP Federation who support core General Practice and help sustain its future. The Federation are facilitating and supporting the recruitment of these new roles within our Primary Care Network.

As a compassionate, motivated, and collaborative care coordinator, you will be joining a thriving existing PCN team of care coordinators, social prescribers, health and wellbeing coach, mental health practitioners, first contact physiotherapist, and pharmacy team.

  • Provide personalised support to individuals, their families, and carers to support them to be active participants in their own healthcare; empowering them to manage their own health and wellbeing and live independently
  • Support patients to establish and attain goals that are important to the patient and use health coaching and motivational techniques to support patients in managing their wellbeing
  • Develop collaborative relationships and work in partnership with health, social care, and community and voluntary sector providers and multidisciplinary teams to holistically support patients wider health and wellbeing, public health, and contributing to the reduction of health inequalities
  • Working with multiple members of the PCN team to coordinate the booking of appointments to ensure a good patient experience
  • Help patients to manage their needs through answering queries, making and managing appointments, and ensuring that they have good quality written or verbal information to help them make choices about their care
