Job summary
To work as an autonomous practitioner, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the patient population. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients
The advanced nurse practitioner will be responsible for a number of clinical areas such as health promotion, minor illness, as well as supporting the management team in the reviewing of clinical policy and procedure.
Main duties of the job
We are looking for an Advanced Nurse Practitioner who is passionate about providing excellent care. Is compassionate and a real team player. You will be working alongside an approachable and supportive GP Partner every session.

Although you will be part of the Moorlands Rural PCN team you will mainly be based at Werrington Village Surgery. We are a caring, friendly and supportive looking to recruit a dedicated team member to join us to help maintain our high standard of patient care. We are a forward-thinking training practice caring for 8,500 patients. The practice has a village feel and is supported by an active Patient Group.

Induction Training

All staff will be required to partake and complete mandatory training as directed by the training coordinator, as well as participating in the practice training programme. Staff will also be permitted (subject to approval) to undertake external training courses which will enhance their knowledge and skills, progress their career and ultimately, enable them to improve processes and service delivery.

Service Delivery

Primary Responsibilities
The following are the core responsibilities of the Advanced Nurse Practitioner. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

Develop, implement and embed health promotion and well-being programmes.
Implement and evaluate individual specialised treatment plans for chronic disease patients.
Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health.
Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.
Request pathology services as necessary.
Process and interpret pathology and other test results as required.
Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required.
Maintain accurate clinical records in conjunction with extant legislation.
Ensure SnoMed codes are used effectively.
