Job summary
An exciting opportunity has arisen to work within Newcastle Central PCN. You will work to contribute to improve health outcomes in long term conditions as a member of the multidisciplinary primary care team within the PCN practices. You will provide specialist knowledge and advice on pharmaceutical matters, including cost-effective prescribing and clinical guidelines for disease treatment, and to also provide evidence-based education and training for colleagues in partnership with the practice. You will provide advice and support for patients with self-limiting conditions. You will work to improve communication about medication-related issues between the practice and other care providers and to lead on implementation of safe and efficient repeat prescribing systems. This is a new and exciting role for you to develop and strive to support the needs of the PCN going forward.

Main duties of the job
To contribute to improve health outcomes in long term conditions as a member of the multidisciplinary primary care team within the PCN
practices.
To provide specialist knowledge and advice on pharmaceutical matters, including cost-effective prescribing and clinical guidelines for disease treatment, and to also provide evidence-based education and training for colleagues in partnership with the practice.
To provide advice and support for patients with self-limiting conditions.
To play a key role in helping to deliver the new network service specifications, particularly the delivery of the structured medication reviews and medicines optimisation and enhanced health in care homes.
To work to improve communication about medication-related issues between the practice and other care providers.
To lead on implementation of safe and efficient repeat prescribing systems.
About us
Newcastle Central PCN are a group of practices working together to focus on local patient care. We are a small multi-disciplinary team, covering 6 practices across Newcastle Under Lyme, covering a total population of circa 40,000.
We have a strong leadership, led by our PCN Clinical Director and a supportive management team.
We are looking at expanding our team which also includes care co-ordinators, pharmacy technicians, social prescriber, dietician, mental health practitioners and paramedics.
We are a forward thinking and innovative PCN who utilises to the full, the skills and experience of our team members. We have a flexible approach and will consider part time or job share opportunities.
We are supportive of professional development and pride ourselves on strong peer support for these new roles whilst working in a supportive and friendly environment.
We work collaboratively with our local GP Federation who support core General Practice and sustaining the future for General Practice. The Federation are facilitating and supporting the recruitment of the new roles within our Primary Care Network.
