Job summary
An exciting opportunity has arisen to work within Moorlands Rural PCN. You will be undertaking clinical, technical, and administrative tasks. You will support the role of PCN Clinical Pharmacists in the delivery of clinical pharmacy services. You will perform medicines improvement work to maximise safe, cost-effective best practice in prescribing under the supervision of the GP or Clinical Pharmacist. You will assist with medication safety monitoring systems (e.g., high-risk drugs). You will liaise with clinicians and administrative staff to resolve medicine-related queries. You will monitor practices prescribing local and national prescribing policies. You will support the Clinical Pharmacists in Structured Medication Reviews (e.g., organise necessary monitoring tests before SMR). You will undertake patient-supporting roles to ensure effective medicines use through shared decision-making and you will also support medicine reconciliation of patients whose care is transferred back into primary care in a timely and effective manner liaising with patients and other providers to ensure patients receive appropriate medication post discharge. This role is expected to develop and evolve over time as the successful post holder gain experience.

- Registration with the General Pharmaceutical Council (GPhC)
- BTEC / NVQ Level 3 of equivalent in pharmaceutical science / pharmacy services

- Experience of dealing with a range of clinical conditions
- Expericence of working within a primary care, community or hospital setting
- Good clinical pharmacy knowledge including terminology
- Relevant advanced theoretical and practical knowledge of Primary Care Networks, General Practice and evidence based medicine
- Able to interpret and analyse prescribing data
- Works effectively independently and as a member of a team
- Flexible approach

About us
Moorlands Rural PCN is a group of practices working together to focus on local patient care.We are a small multi-disciplinary team, covering 7 practices across Cheadle, Alton, Walterhouses, Werrington, Tean and the Blythe Bridge areas, covering a total population of 37000.
We have a strong leadership, led by our PCN Clinical Director and a supportive management team.
We are looking at expanding our Pharmacy team which currently includes 4 Clinical Pharmacists and 2 Pharmacy Technicians.
We are a very forward thinking and innovative PCN who utilises to the full, the skills and experience of our team members. We have a flexible approach and will consider full time / part time or job share opportunities.
