Job summary

EACH is offering a unique opportunity to join our team as a Symptom Management Nurse, with a structured 18 month development programme designed to support you in progressing to a Band 7 Clinical Nurse Specialist (CNS) role.

We are looking for an experienced, registered Children, Adult or Learning Disabilities nurse to join us as Symptom Management Development Nurse.

You will be a point of contact for symptom management issues for professionals and existing teams in health, social care and education services across the statutory and voluntary sectors.

Main duties of the job

The EACH Symptom Management Nursing Service (SMNS) provides a 24/7 specialist nursing, support and advice service for children with life-threatening illnesses and complex health care needs living across East Anglia, working with the RAaFT (Regional Advice and Facilitation Team) for Childrens Palliative Care.

The SMNS provides expert symptom management, care and support to children across the whole care pathway. Acting as a point of contact for symptom management issues for professionals and existing teams in health, social care and education services across the statutory and voluntary sectors.

Actively participating in the EACH 18 month training pathway, working through competencies focused on developing skills to the level of a Clinical Nurse Specialist (CNS)

Job description

Job responsibilities

  • Actively participate in the development competency program, working through the competencies at an appropriate pace to progress to role of Clinical Nurse Specialist (CNS) (RCN Caring for Infants, Babies, Children and Young People Requiring Palliative Care 2025).
  • Act as a point of contact for symptom management advice for all children and young people across the continuum of care and palliative care pathway in the full range of settings (home, hospital, hospice, school).
  • Manage a case load within a designated geographical area in EACH, supported by the Symptom Management Nursing Service (SMNS) CNS.
  • Develop the ability to provide comprehensive, holistic, evidence based nursing care to children and young people with complex palliative care and symptom control needs.
  • Develop the ability to undertake clinical assessment / consultation using face to face, telephone and virtual IT methods as required with CNS support.
  • With support from the SMNS CNS, develop and regularly review symptom management plans. Over time, you will build the skills needed to write these plans independently before moving into the CNS role.
  • Work as part of the local team to provide local on call symptom management service as required.
  • Working initially alongside a buddy in the provision on the regional out of hours 24/7 palliative care on call nursing service. Working towards contributing to on call rota, extra to contracted hours with additional payment.
  • Meet, both directly and indirectly through others, the comprehensive and specific complex communication needs of babies, children, young people (BCYP) and their families.
  • Provide support for care coordination/key working/lead professional role responsibilities.
  • Support statutory and voluntary services to provide a co-ordinated multidisciplinary approach to the provision of palliative care for BCYP.
  • Deliver learning and development to enhance knowledge, skills and confidence and increase capability in universal and core services across health, social care and education and in EACH.
  • Be aware of new approaches to care and contribute to improvements in paediatric palliative care.
  • Participate alongside CNS in clinical audit and evaluation of ways of working and service efficacy, supporting clinical quality initiatives as agreed with the Matron and Head of Nursing.

  • Ability to participate in the local on-call rota when required to deliver urgent or end of life care in the hospice or community. An on call allowance is payable for each session.
  • Ability to participate to in regional on call rota initially with buddy support as per the development programme.
