Location: Bromley

Hours: 22 hours per week - 12-month fixed term contract with additional opportunities available if you want to work full time 

Salary: £49,178 - £55,492 pro rata (inclusive of HCA, salary dependent upon experience)

This post is an innovate, dynamic, autonomous role to support the development of the Virtual Ward Project situated in the London Borough of Bromley. Health & Social Care organisations in Bromley have agreed to work together to enhance and improve the range, quality and effectiveness of services available to local people. Working as a single system, the One Bromley Local Care Partnership intend to develop a strategy in common and jointly direct the resources, skills and assets available within Bromley in a coordinated way to achieve better outcomes. This service will provide care, monitoring and treatment at home to patients who otherwise would have been in hospital. The purpose is to either prevent avoidable admissions into hospital or support early discharge out of hospital.

St Christophers has services spanning the London boroughs of Bromley, Croydon, Lambeth, Southwark and Lewisham, and are connected to partner health and social care organisations in all the boroughs we support. St Christophers CARE, provides learning and development to the communities we support, and we have academic links with a number of universities, nationally and internationally. This is an exciting opportunity for an experienced community palliative care nurse to join a new initiative within the St Christophers Bromley Community Palliative Care Team.
