Job summary
Job type:30 hours per week over 4 days.
Salary:Based on 30 hours £21,447 £22,965. Full-time equivalent £26,809 £28,706.
Location:The successful candidate will be required to travel between Stratford PCN Practices.Due to the geography and nature of the role a clean UK driving license and full use of car is essential.
Main duties of the job
Stratford Central PCN seek an enthusiastic and proactive Care Coordinator to join their team and deliver care to their patient populations. The Care Coordinator role is a new and evolving role within NHS Primary Care Networks. The post holder will require a flexible approach to work, the role will consist of some focussed project work as well as general core support to the PCN.
The post holder would ideally have good knowledge of community or primary care sector and strong IT skills as the role will be key to practice searches and audits.

They will support the Clinical directors in coordinating all key activity including improving access to services, providing advice and information and ensuring health and care planning is timely, efficient and patient centered. They will work closely and in partnership with the Social Prescribing Link Workers and support delivery of the comprehensive model of care to reflect local priorities, health inequalities or population health management.

- Access to own transport and ability to travel across the locality on a regular basis, including to visit people in their own homes.

- Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement (including unpaid work).
- Experience of supporting people with their mental health.
- Training in motivational coaching and interviewing or equivalent experience
