Hours: Part-time 20 hours per week (MondayFriday, 9:00am1:00pm)

Base Location:Base Location: For the successful candidate, the PCN will consider hybrid working arrangements, which can be discussed at the interview stage. The role will be primarily based at Fenny Compton Surgery. The successful candidate must be willing to travel between all PCN practices as required to meet service needs. Due to the geography and nature of the role, a full UK driving licence and access to a car are essential.

Main duties of the job

Warwickshire East PCN is looking to recruit a Clinical Pharmacist who is passionate about delivering high-quality care to patients.

This role supports improved patient health outcomes and the efficiency of the primary care team by providing direct, accessible, and timely medicines expertise, including face-to-face consultations.

The Clinical Pharmacist will proactively take responsibility for workload relating to medicines optimisation, improving patient care and safety while enabling clinical staff to focus more time on direct clinical care.

Job responsibilities

  • Support patients to get the best from their medicines, including switching medications in line with agreed protocols.
  • Improve repeat prescribing processes in general practice, including promotion of repeat dispensing and online ordering.
  • Minimise clinical risk and work to reduce wasted medicines.
  • Work as part of a multidisciplinary team in a patient-facing role, clinically assessing and treating patients using in-depth clinical knowledge.
  • Be directly responsible for the care and prescribing of defined cohorts of patients.
  • Deliver and develop pharmacy-led medicines optimisation services within general practice and across the wider multidisciplinary team.
  • Ensure practices remain up to date with medicines management guidance and best practice.
  • Lead and promote medicines optimisation within specialist areas, ensuring best value, reduced harm, and improved patient outcomes.
  • Lead and deliver evaluation and audit of service delivery to maintain and improve service quality.
  • Work in line with Medicines Optimisation principles, national guidelines, and the latest evidence base.

Preference will be given to candidates who are CPPE pathway trained, or candidates must be willing to undertake CPPE training and enrol within six months of employment if not already completed.
