Salary:Starting from £46,696 per annum (depending on experience)Hours:Full Time 37.5 hours per weekLocation:Across Kenilworth & Warwick PCN Practices

You will be required to travel between the following practices:

  • Abbey Medical Centre
  • Avonside Health Centre
  • Castle Medical Centre
  • Chase Meadow Health Centre
  • Priory Medical Centre

Due to the nature of the role, a full UK driving licence and access to a vehicle is essential.

Main duties of the job

Kenilworth & Warwick PCN are seeking an experienced and motivated Enhanced Nurse to work as part of the multidisciplinary team (MDT) across five practices, supporting care homes and the frail elderly population.

This is a rewarding opportunity to work autonomously at an enhanced level of practice, delivering high-quality, patient-centred care within the community and care home settings.

Job responsibilities

  • Work collaboratively as part of the PCN multidisciplinary team
  • Support care homes and the frail elderly population across the PCN
  • Undertake comprehensive assessments of patient need
  • Develop, implement and evaluate complex care plans in patients homes and/or care homes
  • Consult with patients, families and the wider multi-professional team
  • Provide supervision and support to trainee nurse associates, nurse associates and general practice nurses
  • Maintain accurate, high-quality patient records and manage referral/care pathway databases
  • Participate in clinical audits and research projects
  • Develop, update and implement local practice protocols, guidelines and procedures
  • Build effective working relationships with PCN member practices and care homes
  • Develop productive relationships with GPs, consultants, district nurses, social services, NHS Trusts and voluntary sector partners
  • Demonstrate initiative and creativity in problem solving

  • Experience managing frail elderly patients
  • Knowledge and understanding of clinical governance
  • Ability to work independently and make autonomous clinical decisions within scope of practice
  • Experience working in a highly demanding environment
  • Evidence of ongoing Continuing Professional Development (CPD)
  • Excellent written and verbal communication skills
  • Ability to reflect on own practice

Desirable

  • Awareness of current national and local health and social care agendas, particularly within Primary Care and PCNs
  • Knowledge of audit and research, with experience analysing findings to improve practice
  • Understanding of policy developments related to primary care delivery including:
  • General Practice
  • GMS/PMS contracts
  • Clinical Governance
  • Quality & Outcomes Framework (QOF)

Qualifications

  • Registered Healthcare Professional (NMC or HCPC)
  • Experience working across services that enhance the core general practice offer
  • Working at an enhanced level of practice as outlined in the Primary Care and General Practice Nursing Career and Core

Desirable

  • Non-Medical Prescribing qualification
  • Relevant Masters degree (Level 7 / 180 credits)
