Main duties of the job
Join a forward-thinking, patient-centred Primary Care Network making a real difference across Kenilworth and Warwick. We are seeking an experienced Advanced Clinical Practitioner to work across two dynamic practices Abbey Medical Centre and Avonside Health Centre delivering high-quality, evidence-based care and leading clinical innovation within the PCN team.

As an ACP, youll work autonomously and collaboratively within our multi-disciplinary team to:
- Assess, diagnose, and manage patients with both undifferentiated and complex presentations
- Prescribe and review medication, using advanced clinical reasoning and evidence-based practice
- Lead and contribute to PCN projects aligned with national priorities including health inequalities, long-term conditions, learning disabilities, and vaccination programmes
- Support service development, staff education, and continuous quality improvement through leadership, teaching, and audit activities
Youll bring advanced clinical knowledge, critical thinking, and a passion for patient-centred care to shape the future of our local primary care services.

- Educated to Masters level (or equivalent) in Advanced Clinical Practice
- A qualified Independent Prescriber (IP)
- Experienced in primary care, with confidence managing autonomous clinical decision-making
- Skilled in EMIS and confident with digital tools for effective patient care
- A natural leader, educator, and team player with excellent communication and problem-solving skills
