Administrative Care Coordinator | Kenilworth & Warwick Primary Care Network

Salary: £25,112 per year.
Job type: Full time. 37.5 hours per week. Monday - Friday.
Location: The successful candidate will work across all Kenilworth & Warwick PCN practices. Due to the geography and nature of the role a clean UK driving license and full use of car is essential.
Main duties of the job
Job summary:To support the Kenilworth and Warwick Primary Care Network Management team on effective administration and management of meetings, calendars, correspondence, actions, and project delivery.
This role will work under the direction of the PCN Clinical Director, PCN Strategic Manager and PCN Operations Manager to support them manage and develop strategy and process.

Job responsibilities
Duties and Responsibilities:
Arrange meetings
Take minutes in meetings
Ensure actions from meetings are completed In a timely manner
Send out meeting agendas and associated papers out documents
Act as a contact point for the PCN
Send/receive e-mail's on behalf of the Clinical Director and PCN Strategic Manager where appropriate
Complete actions for Clinical Director and PCN Strategic Manager where appropriate
Keep Teams groups and documents updated
Provide admin support to other leadership positions within the PCN where required
Work closely with the Social Prescribers and Health and Wellbeing Coaches
Supporting the PCN leadership team to look at population health intelligence to help proactively identify and deliver personalised care
Work with the GPs and other primary care professionals within the PCN to help support them in identifying and managing a caseload of patients, and where required and as appropriate, refer people onward to other health professionals within the PCN
Raise awareness within the PCN of shared decision-making and decision support tools
Raise awareness of how to identify patients who may benefit from shared decision making and support PCN staff and patients to be more prepared to have shared decision-making conversations.
