Job summary

We are encouraging applications from motivated Advanced Clinical Practitioner: Paramedics, Nurses, and Physicians Associates, who are looking for a new opportunity and the chance to support an innovative project. Our patient focused Access Hub aims to help manage acute, on-the-day demand to improve access to enhanced healthcare services within the Leamington Spa Primary Care Network.

What will be expected of you:

You will provide care for patients from initial history taking and clinical assessment through to the diagnosis, treatment and evaluation of care.

You will demonstrate critical thinking in the clinical decision-making process, including assessment and diagnostic skills, leading to the delivery of safe care for all patients.

You will work collaboratively within a dedicated healthcare team to meet the needs of our patient population, whilst adhering to set policy and procedures.

You will provide a holisitic and clinical service, implementing agreed management plans and follow approved protocols as appropriate.

You may be expected, on a typical day, to carry out home visits and consult with patients via telephone, video and face to face via booked appointments.

You will also review and act appropriately on blood results and referrals correspondence regarding patients.

This list of duties is not finite and will grow as the project develops.
