Work location:The successful applicant will be required to travel between the Stratford PCN practices. Due to the geography and nature of the role a clean UK driving licence and full use of car is essential.

An exciting opportunity has arisen to join the Stratford Central PCN as a Social Prescribing Link Worker. We are a forward thinking PCN of 4 practices and are working together to extend the social prescribing provision to our patients in the local area. We are looking for a proactive caring individual to join our small and friendly Health and Wellbeing team. Main duties of the job
Social Prescribing is a free service to help people improve patients general health and wellbeing in a holistic way. Through using non-medical sources of community support, Social Prescribing offers the chance to find out about activities that can improve health and wellbeing, including advice and information services, community groups, leisure activities, lunch clubs, self-help groups, specialist interest groups, sporting activities, and lots more.
As a link worker you will be working closely with the Health and Wellbeing Team, taking referrals from the surgeries, placed Based Teams, and other third-party roles.
A link worker will develop relationships in the health and social care sector, working closely with our Health and Wellbeing Coach, Care Coordinator and Clinical Pharmacists, and supporting individuals to access voluntary and community opportunities.
You will be responsible for providing non-clinical personalised support to individuals and their families, in order to empower them to take control of their own health and wellbeing, give them time and focus on what matters to me, in order to holistically improve an individuals health and wellbeing.

South Warwickshire GP Federation seek an enthusiastic and proactive Social Prescriber to develop as General Practice Social Prescribing Link Workers on behalf of the Stratford Central PCN which is made up of the following practices:
- Trinity Court Surgery
- Rother House Medical Centre
- Bridge House Medical Centre
- Bidford Health Centre
Salary is dependent on experience and qualifications and includes NHS pension. Due to the geography and nature of the this role a clean UK driving licence and full use of care is essential.
