Main duties of the job

Leamington North and South Primary Care Networks are committed to recruit clinicians from a variety of backgrounds to work within an innovative access hub. Our patient focused Access Hub aims to help manage acute, on-the-day demand to improve access to enhanced healthcare services within Leamington.

We are looking for candidates from a nursing, paramedic, physicians associate (including apprentice) background, that are committed to personal development in order to create a multi-disciplinary team and promote shared learning.

Job description

Job responsibilities

Provide first point of contact care for patients presenting with undifferentiated, undiagnosed problems by utilising history taking, physical examinations and clinical decision-making skills to establish a working diagnosis and management plan in partnership with the patient (and carers as applicable)

Identify, signpost, or refer patients at risk of developing long-term conditions, preventing adverse effects on the patients health

Support the management of patients conditions through offering specialised clinics following appropriate training including (but not limited to) family planning, baby checks, COPD, asthma, diabetes, and anticoagulation

Develop integrated patient centred care through appropriate working with the wider primary care multi-disciplinary team and social care

Provide health / disease promotion and prevention advise alongside analysing and actioning diagnostic test results

Provide routine care to patients as required in accordance with and promoting clinical based evidence. Prioritise health issues and intervene appropriately

Examine, assess and diagnose patients and provide clinical care/ management as required

Triage patients and provide the necessary treatment during home visits

Collect pathology specimens as required

Maintain accurate clinical records including read codes in conjunction with legislation

Review the effectiveness of the treatment provided, making changes where necessary to improve patient outcomes

Recognise, assess and refer patients presenting with mental health needs

Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice)

Adhere to the relevant patient group directives and local clinical pathways

Support the clinical team with all safeguarding matters, in accordance with local and national policies

Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

Secondary responsibilities

Support the practice audit programme, undertaking audits when necessary

Support junior members of the team including trainees, providing guidance when necessary

Participate in local initiatives to enhance service delivery and patient care

Support and participate in shared learning within the practice

Participate in the review of significant and near-miss events applying a structured approach, i.e. root cause analysis (RCA)

Take personal responsibility for own learning and development, including the requirement to maintain currency, achieving all targets set in their Personal Development Plan (PDP)

Follow recommended annual appraisal & CPD as per requirements of the Faculty of Physicians Associates (FPA).

Qualifications

Essential

  • Post graduate diploma or degree (physician associate)
  • Member of Physician Associate Managed Voluntary Register
  • Passed UK PA National Re-certification exam every 6 years
  • Willingness to work flexible hours when required to meet work demands
  • Aware of data protection (GDPR) and confidentiality issues particularly within a PCN
  • Meets DBS reference standards and has a clear criminal record, in line with the law on spent convictions
  • Full UK Driving Licence, with access to own transport and ability to travel across the locality on a regular basis, including to visit people in their own homes

Desirable

  • Non-medical prescribing qualification

Experience

Essential

  • Two years experience post undergraduate qualification
  • Experience in managing a designated caseload using theoretical and practical experience, completing assessments, planning, implementing interventions, and evaluating outcomes aligned to care plans

Desirable

  • Experience of working within a primary care setting
  • Competent use of EMIS
  • Triage experience
  • Working towards advanced clinical practitioner status
  • Cognitive behavioural and motivational interviewing approaches/ skills
