Main duties of the job
Leamington North and South Primary Care Networks are committed to
recruit clinicians from a variety of backgrounds to work within an innovative
access hub. Our patient focused Access Hub aims to
help manage acute, on-the-day demand to improve access to enhanced healthcare
services within Leamington.
We are looking for candidates from a nursing, paramedic, physicians
associate (including apprentice) background, that are committed to personal
development in order to create a multi-disciplinary team and promote shared
learning.

What makes our approach and this role different? We are committed to

In this role, you will deal with acute presentations of minor injury
and illness; assessing, diagnosing and implementing treatment. To ensure equal
access to healthcare for all, there will be opportunities to conduct home
visits for our housebound population.
You will hold an undergraduate degree and be two years post qualification.
You will preferably have a postgraduate degree along with an
Independent Prescribing Qualification and prior experience working in primary
care.

- Experience in managing a designated caseload using theoretical and practical experience, completing assessments, planning, implementing interventions, and evaluating outcomes aligned to care plans
Desirable
- Experience of working within a primary care setting
- Competent use of EMIS
- Triage experience
- Working towards advanced clinical practitioner status
- Cognitive behavioural and motivational interviewing approaches/ skills
Qualifications
Essential
- BSc in a training programme approved by the College of Paramedics
- Health & Care Professions Council (HCPC) registration
- Framework for Higher Education Qualification (FHEQ) Level 5/6/7 or Scottish Credit and Qualifications Framework (SCOF) Level 8/9/10/11
- Willingness to work flexible hours when required to meet work demands
- Demonstrable commitment to professional and personal development

- Aware of data protection (GDPR) and confidentiality issues particularly within a PCN
- Meets DBS reference standards and has a clear criminal record, in line with the law on spent convictions
- Full UK Driving Licence, with access to own transport and ability to travel across the locality on a regular basis, including to visit people in their own homes

- Pre-reg MSc in a training programme approved by the College of Paramedics
- Non-medical prescribing qualification
