Job summary
Join a dynamic, multidisciplinary team delivering expert clinical and prescribing support to individuals affected by drug and alcohol use. Main duties of the job
Key Responsibilities:
- Undertake service user centred assessments, care planning and delivery, monitoring, evaluation and reviews in practice and outreach settings.
- Work in conjunction with the multi-disciplinary team (MDT) in developing and delivering prescribing treatment modalities for service users.
- Prescribe cost effectively and according to best practice and provide prescribing information and advice to Pharmacists, Shared Care GPs and hospital teams.
- Support colleagues (Dr, IP) through individual clinical reviews as requested to support discussion and the development of a safe and effective treatment plan.
- Liaise with all agencies delivering services to or sharing care of clients including but not limited to sharing care with GPs for Service Users in Shared Care Services.
- Lead on complex appropriately assigned alcohol detoxification assessments for individuals who have decompensated liver disease and tolerance to diazepam.
- Provide prescribing information and advice to Pharmacy and Shared Care GPs.
