Job summary

  • To work as part of a multi disciplinary team on the Wellbeing unit delivering high-quality evidence-based holistic palliative care to patients and their families ensuring all care is provided with compassion dignity and respect

  • Undertake individual patient assessments formulating goal setting and plans of care

  • Be responsible for agreed service coordination and quality tasks, for example data collection, audits, attendance at multidisciplinary meetings.

  • To lead creative therapies within the Wellbeing unit with support from volunteers and external services.

  • Support and supervise the work of Support Workers, Students and Volunteers

  • The post holder will provide individualised holistic enablement focused palliative rehabilitation within their scope of practice.

  • Partake in the on-call rota.

  • Contribute to and participate in organisational service development.

  • Work across the clinical services, including Hospice at Home when required, to ensure service delivery.

Main duties of the job

  • To provide skilled care under the direction of the Team Leader that is evidence based and best practice.

  • To lead the process of admission to service ensuring all relevant patient records are completed within the agreed timeframes and according to statutory requirements

  • To lead patient groups and support colleagues in the delivery of group work

  • To undertake patient assessments in the Wellbeing setting and the patients home

  • To formulate goal setting and individual care plans and to be able to implement, evaluate and amend these plans

  • To plan, deliver and evaluate evidence based rehabilitative care for patients and their families

  • To develop communication skills to promote beneficial therapeutic relationships with patients and their carers.

  • To ensure patients receive the Right Care at the Right Time in the Right Place including Advance Care Planning (ACP) and Advance Clinical Management Planning (ACMP)

  • To liaise with the multidisciplinary team in both the Hospice and the community setting including Palliative Consultant, District Nurse, Social Workers, GPs, and any other agencies involved in patient care.

  • Maintain and support the efficient use of physical and / or financial resources.

  • To monitor patients condition and report significant changes to the Team Leader, GP and/or emergency services as appropriate.

Qualifications

Essential

  • NMC or HCPC Registration
  • Documentary evidence of continuing professional development

Desirable

  • Evidence of recent academic achievement relating to palliative care long term conditions
  • Counselling skills communication skills training

Experience

Essential

  • At least 2 years post-qualified experience in adult care.
  • Experience of supervising/mentoring junior staff

Desirable

  • Experience of caring for patients with long-term conditions
  • Experience of palliative and end of life care
  • Experience of using creative therapies within a healthcare setting
