Team Leader - Wellbeing Services

Job summary

  • To have overall responsibility for the leadership and co-ordination of the Wellbeing services, including the Complementary Therapy service.
  • To serve as an inspirational leader and ambassador of the hospice in all interactions both within the organisation and with external stakeholders.
  • Deliver high-quality, evidence-based holistic palliative care to patients and their families, ensuring all care is provided with compassion, dignity, and respect.
  • Ensure services are delivered safely, efficiently, and effectively by supporting a well-trained and competent workforce, with appropriate monitoring of outcomes and quality standards.
  • Actively contribute to the ongoing development and improvement of clinical services across the organisation.
  • Partake in the out of hours on-call rota.

  • Work across the clinical services, including Hospice at Home when required, to ensure service delivery.

Main duties of the job

    • Oversee the delivery of holistic care and associated activities across wellbeing and complementary therapy services, ensuring alignment with relevant professional standards, clinical guidelines, and best practices.
    • Lead the planning, delivery, and evaluation of evidence-based rehabilitative care
    • Plan, conduct, and supervise comprehensive holistic assessments for all patients
    • Lead patient group sessions and support colleagues in the delivery of group-based activities
    • Facilitate patients to receive the Right Care at the Right Time in the Right Place including Advance Care Planning (ACP) and Advance Clinical Management Planning (ACMP)
    • To develop the service to become a community hub, integrated within the community and impacting on the social/public health concerns within the Rossendale Valley
    • Foster good working relationships and effectively communicate with members of the multidisciplinary team, users of the service and other healthcare professionals working externally
    • Lead by example in demonstrating excellent communication skills, offering support with compassion and professionalism, and managing emotionally challenging situations in a calm, respectful, and constructive manner.
    • Maintain and support the efficient use of physical and / or financial resources.

Essential

  • Qualified Health Care Professional and registered with a UK regulatory professional body e.g. HCPC or NMC
  • Documentary evidence of continuing professional development

Desirable

  • Evidence of recent academic achievement relating to palliative care/long-term conditions
  • Counselling skills/communication skills training

QUALITY

Essential

  • Ability to work proactively, independently and on own initiative
  • Ability to work as part of a team, demonstrating flexibility, punctuality and reliability
  • Be empathetic, compassionate, resilient, and adaptable to change

Experience

  • At least 2 years post-qualified experience in adult care.
