Job summary

We are looking for a Pharmacist to join our Hinckley Central PCN team, working in The Centre Surgery

We are looking for someone who can work 30 hours per week, either over 4 or 5 days.

Ideally we are looking for somebody who has already completed the CPPE Course.

If you have not completed your CPPE, this should not stop you from applying, but please be aware that there would be a requirement for you to enrol onto the course.

Hinckley and Bosworth Medical Alliance is a GP Federation that supports 12 GP practices across the Hinckley and Bosworth locality, which form 3 Primary Care Networks.

We employ in excess of 50 PCN staff across a number of clinical grades, and continue to grow. This includes a team of 19 Pharmacists and Technicians.

As a rapidly growing Federation, we have structures in place to ensure that all staff have clinical mentors and are supported in their professional development.

Starting salary of £48,928 for a non-prescribing pharmacist. The starting salary for a prescribing pharmacist is £52,190. This is based on full time hours. Any salary will be pro-rata for the working hours accordingly.

We operate a salary scale, with increases available upon successful completion of probation as well as at your first annual appraisal, post appraisal (approximately 18 months in post).

Main duties of the job

Job Summary

To participate in the provision of a Clinical Pharmacy Service to our member GP Practices defined within the Primary Care Network geographical location;

  • Provision of routine clinical medicines reviews
  • To review and reduce were possible inappropriate poly-pharmacy and wasteful prescribing
  • Reconcile patients medicines following hospital discharge
  • Work with patients and community pharmacists to ensure patients receive the medicines needed post discharge
  • Contribute to reductions in medicine related hospital admissions and re-admissions
  • Interface with community and hospital pharmacy colleagues
  • Manage the repeat prescribing reauthorisation process
  • Manage patients and health care professionals medicine queries
  • Increase quality and safety of prescribing through mechanisms such as audit
  • Manage Primary Care Network practice formularies to improve quality, safety and cost
  • Implement drug withdrawals and alerts
  • Work with primary care professionals and patients to implement NICE and other evidence based guidelines
  • Run clinics where appropriate to meet the needs of the Primary Care Network
  • Contribute to multi-morbidity reviews and multidisciplinary reviews
  • Act as a source of medicines information for all of the practice team and patients

About us

The Centre Surgery is part of the Hinckley Central PCN, and has over 5,000 patients, that the successful applicant will help and support.

We have an established team of pharmacists throughout the PCN, whom have all completed their CPPE programme and most having also completed their IP.

The successful candidate will enrol into the CPPE programme (if they have not already completed it).

We will also support any successful applicants to complete their Independent Prescribing Qualification, should they not already hold it and wish to upskill. This will only be considered after the successful completion of the CPPE.
