Job summary

Do you have experience of working in healthcare, have great organisation and patient facing skills? Are you able to demonstrate good IT skills including Excel, Word, and Outlook?

You will be part of the Care Coordinator Team, you will need to have a flexible approach and adapt with the role as it evolves. You will have excellent communication and administrative skills to enable the collaboration needed to deliver services and support development at network level.

Main duties of the job

  • Provide administrative support to the weekly MDT and other meetings
  • Provide simple reception services alongside other aspects of the role, including checking patients in for appointments and answering basic queries
  • Support delivery of QOF, incentive schemes, IIF and other quality or cost effectiveness initiatives
  • Provide administrative support to the home visiting team, including coordinating visits, liaising with practices, patients, secondary care and community services
  • Uploading the appointment books to EMIS web
  • Liaising with practices to support in the delivery of the enhanced access contract
  • Cover for the rest of the care coordinator team during periods of absence and increased workload, e.g. seasonal vaccine campaigns

Canterbury Integrated Healthcare supports the 7 Canterbury practices and their PCNs covering a patient population of around 100,000 and we have worked together for many years.

The local care vision in East Kent is for localities to develop a model of care that enables people to live as independently as possible by delivering high quality, person centred care that integrates hospital, social care, community and voluntary services around primary care networks of GP practices.

The vision for Canterbury is for the two PCNs to work together to achieve excellent patient care through integrated pathways accessed close to home via local GP practices.

The Canterbury PCNs will achieve this by delivering primary care at scale, including:

  • Engagement from all practices within the City in the Integrated model of care
  • Partnership working with other PCNs, NHS Trusts, community providers and the voluntary sector
  • A comprehensive workforce strategy including engagement with the new medical school for Kent and Medway and the new Additional PCN roles.

We aim to work collaboratively with other healthcare providers and support organisations, to enable more patients to be treated in a primary care setting, closer to home.

Job responsibilities

The Care Coordinator Team is primarily responsible for supporting the Canterbury PCNs in the collaborative delivery of services and supporting the care for cohorts of patients. The main focus of the role will be on coordinating the care of identified cohorts of patients and ensuring PCN services are delivered effectively. The Care Coordinator might be required to liaise with patients and, if appropriate, their carers, before or after the patients consultation with a clinician or other healthcare professional. You might also be given a caseload of identified patients and be required to ensure that their changing needs are addressed by taking into account local priorities, health inequalities and/or population health management risk stratification.

The role includes:

  • Working closely with clinical leads and the wider Care Coordinator Team
  • Liaising with health and care professionals practice teams, patients and carers
  • Supporting collaborative working across Canterbury
  • Clinical system searches & reports to identify cohorts of patients and effectiveness of their care
  • Supporting the delivery of PCN services
  • helping to establish and maintain effective systems and processes

The role will evolve and develop in line with emerging needs and priorities of services and the local system.

The role will be based mainly at our premises on Ethelbert Road. It will be necessary to have the ability to travel around Canterbury and the surrounding area, between practices and to be able to work remotely some of the time.
