community-based social prescribing and self-management service which supports
adults aged 18+ to improve their health, wellbeing and independence.

Main duties of the job
Link Workers provide one-to-one support to clients to
identify what matters to them and offer information, advice and support to help
them to:
- Explore options to address non-clinical needs
- Improve their long-term health and wellbeing
- Address health inequalities
This involves working in a multi-disciplinary environment
and develop networks between the voluntary and community, health and social
care, and primary care sectors. As part of this, the Link Worker will lead on
the development of social prescribing initiatives within a specified primary care
network.

Partnership NHS Foundation Trust deliver the social wellbeing service, Live
Well Wakefield. This innovative service aims to improve the wider health and
wellbeing of adult Wakefield residents by promoting independence through social
prescribing and supported self-management interventions.

personalised care support plans. Facilitating empowerment and independence by

relationships with primary care colleagues and other organisations to identify,

to promote health and wellbeing.
- Takes an active role in projects aimed at
improving the health and wellbeing outcomes for specific groups within the
community.
- Work with multi-disciplinary teams and wider
partners to increase take up of preventative activities and services e.g.,
social activities to prevent social isolation.
- Build and maintain knowledge on local
provision to help promote to clients and others.

sessions, networking events and workshops within the area, to improve awareness
of services and increase joint working between VCSE sector organisations,
health professionals and community members.

support for the aims of the work, and champion the needs of local adults.
- Record, monitor, evaluate and analyse work
to ensure that targets are being met, information is recorded accurately, and
best practice is developed.
