Job summary

We are seeking an inspirational Head of Community Services to join our dynamic team. If you are passionate about delivering excellence in community-based palliative care and have a proven track record in leadership and service development, we would love to hear from you.

Main duties of the job

As the Head of Community Services, you will lead, manage, and develop all Hospice Community Services under the supervision of the Director of Clinical Services. Your role will be pivotal in ensuring the delivery of high-quality, patient-centred care that aligns with the Hospice Strategy and the needs of our local communities.

You will provide strategic leadership and operational management for a range of services, including:

Community Clinical Nurse Specialists (CNS)

Fast Track Home Care Domiciliary Service

Wellbeing Services

New Community Outreach Initiatives

Key Responsibilities:

  • Drive the development and implementation of community services that meet the needs of patients, families, and carers, in line with national and local healthcare policy.
  • Provide management and leadership to a multidisciplinary team, fostering a culture of collaboration, innovation, and excellence.
  • Offer specialist palliative care advice to internal and external stakeholders, including professional colleagues, patients, and families.
  • Build and maintain strong relationships across the Hospice and with external partners, ensuring an integrated approach to care delivery.
  • Monitor and evaluate service performance, identifying opportunities for improvement and ensuring compliance with clinical standards and regulatory requirements.

About You: To succeed in this role, you will have:

  • A relevant clinical qualification (e.g., nursing, allied health professional) with specialist knowledge of palliative care.
  • Proven experience in leading and managing multidisciplinary teams within a healthcare setting.
  • Strong knowledge of community healthcare services and policy frameworks.
  • Excellent communication, organisational, and decision-making skills.
  • A commitment to patient-centred care and the values of hospice care.
